ARTICLE I. LABOR AGREEMENT BETWEEN MARC A. CAPIZZI BOARD SECRETARY/BUSINESS ADMINISTRATOR AND ‘THE LODI BOARD OF EDUCATION FOR 2017-2018 FISCAL YEAR 1. Health Care Coverage ‘The Board shall provid héalth insurange for the Secretary/Business Faminisrotar trough te New ersey Sool Employes Heat Benefits Plan, The bgard will crty the obligation of providing health insurance which Is equal to or better than SEHBP a of July 2, 2016, Pursuant to applicable law and regulation, the employee shall cotribute an amount toward payment of Premiums. ‘Prescription Plan Effective September 1, 2015 the Board entered into the New Jersey ‘School Employees’ Health Benefits Plan for Prescription benefits. ‘The Board shall pay the cost of premiums for prescription coverage. Pursuant to applicable law and regulation, the employee shall contribute an amount toward payment of premiums. Said plan shall be $3 co-pay for generic prescriptions and $10 co-pay for ‘brand name prescriptions. The co-pay amounts for mail order prescriptions shall be $5 for generic prescriptions and $15 for brand name prescriptions. The level of prescription benefits as it relates, to all other aspects of the New Jersey School Employees’ Health Benefits Plan for prescription coverage will be the same as was in effect with the Board's prior prescription plan as of July 1, 2014. Expenses incurred by employees for prescriptions shall not be submitted to the Major Medical Program. ARTICLE II, ARTICLE It1. ARTICME IV, ARTICLE V. 3. Dental plan ‘The board shall provide dental insurance coverage (family plan) from the period July 1, 2016 through June 30, 2017. The per diem rate shall be calculated by tmultipiying the annual salary by 1/260. ‘The Lodi Board of Education shall provide the Board ‘Secretary/Business Administrator with two (2) personal days that shall be non-accumulative, ‘The Board Secretary sall be entitled to up to four (4) days at any ‘time in the event of the death of any ofthe Secretary/Business Administrator's spouse, child, son-in-law, daughter-in-law, father-in- law, mother-in-law, brother, sistor, brother-in-law, sistor-in-la, ‘grandmother, grandfather, stepchild, stepmother, stepfather, stepbrother, stepsister, alternative ifestyle partner, or any other ARTICLE VI. The Board Sécretary/Business Administrator is entitled to thirteen (13) Holidafs as follows: \abor Day Good Friday “Thanksgliing Day ‘Memorial Day Day after Thanksgiving February 19 through February 23 Christmas Day In lieu of State & Federal holiday New Year's Day Observances Indepencence Day “The Board Secretary/Business Administrator is entitled to twelve (11) Recess Days as ‘ollows: November 9 & 10% —Teacher Convention December 26" December 29th - Winter Recess April 9® through April 13th — Easter Recess ‘SALARY ‘The Board Secretary/Business Administrator's salary shall be in ‘accordance with the folowing schedule: 2017-2018 171,684.00 ARTICLE VIII. _ PROFESSIONAL DEVELOPMENT ARTICLE IX. Any modifcations/changes to the contract must have the approval of the Executive County Superintendent prior to Board approval. REVOCATION, The Parties hereto agree that in the event the Business ‘Administrator's certification is permanently revoked, all provisions of this Employment Agreement shall be null and void as of the date of ARTICLE X01, ARTICLE XIII.
