AFFIDAVIT OF UNDERTAKING

I, ISRAEL BISERA BANAAN, Filipino, of legal age, married, and resident

of Block 13, Lot 19, Southview Homes, Macasandig, Cagayan de Oro City,
hereby depose and sayeth:

THAT, I am the respondent in NPS No. X-O5-INQ-13G-00233 now under

investigation at the Provincial Prosecutors Office, Hall of Justice, Cagayan de
Oro City for Reckless Imprudence resulting to Homicide and Damage to
Property.

THAT, I am the driver of the Mitsubishi L-300 involved in the case with
plate no. ZCH-918 and C.R. No. 4032589-1 and O.R. No. 280133162 which is
registered under the name of MASS V GROUP, INC. where I am employed as
Dealer Specialist.

THAT, I promise and undertake to produce and bring the said Mitsubishi L-
300 to the Court at any time that it will be needed for inspection by the Provincial
Prosecutors Office or by the Court where the case may be filed .

That I am executing this affidavit in order to attest to the truth of all the
foregoing for all intents and purposes that it may serve under the law.
