My success couldnt turn into reality without this following person who
with all their help to me in different ways. Above all, to our Almighty God for all
His unending blessings and guidance He showered upon me for the past, for
today, and for the future.

To the Government Service Insurance System Tuguegarao Branch and


MEDICard Philippines Inc. for their support and for the permission to render my
OJT to their respective offices.

To Sir Aris Bangayan and Maam Florida Pulido, our OJT adviser, for their
unending support and advises during many trials when I had my Local and
National On-the-Job Training.

Most especially to my beloved parents whom with all their best faces, the

INTRODUCTION

Internship or on the job training is one way by which we students are


given an opportunity to apply the theories and computations that we have
learned from school. It also helps us to obtain applicable knowledge and skills by
performing in actual work setting. Colleges and universities require their students
to undergo such training within a specific number of hours as part of the
curriculum.

For us students, an OJT or internship program provides opportunities to


go through the actual methodologies of a specific job using the real tools,
equipments, and documents. In effect, the workplace becomes a development
venue for us student trainee to learn more about our chosen field and practice
what we have learn from academy.

On the other hand, a valuable OJT program also profits the companies
who accept trainees. First OJT or intern provides extra manpower for a less
significant labor cost than a regular employee. Most of them are all eager to
learn the ropes so chances are high that they will cooperate.

Employers can use this internship strategy as method in recruiting


employees. Since the trainer or supervisor can follow the trainees progress, he
can gauge based on performance, behavior and attitude if the trainee will make
good recruit after the completion of his internship.

We trainees can bring fresh ideas into the organization. Given the
opportunity to converse our minds freely and without fear, we maybe able to
contribute significantly in brainstorming sessions or research and eventually help
improve the organizations productivity. While training the interns, employers are

in fact also teach their employees to guide the trainees by stretching their
patience, develop teaching skills and make them more sensitive to the needs and
mind set of the younger generation. The course of supervision also teaches them
how to share what they know and be receptive to questions. Hence, the
internship also becomes an avenue in training for future managers of the
company.

Accepting on-the-job trainees can truly be beneficial not only to the


trainees but also to the companies that provide opportunities for this type of
learning. There is wisdom in the front lines. Such training can be investment that
we will be valuable to the company later on. This is also why us trainees should
take our internship seriously as it can become a powerful tool and possibly even
a source of recommendation when they take that big lift from being students to
career professionals.

CHAPTER II
COMPANY PROFILE

History
In 1987, a group of eminent physicians from the most distinguished
medical centers in the country, concerned at the ability of working people to pay
for much needed quality health care, consolidated their resources together to put
up MEDICard Philippines, Inc. MEDICard is a pioneer in the industry. The owners
have been in the health care service for years.

The country had suffered badly during the waning years of Marcos rule.
Health costs were rising, but incomes were falling. Even middle income families
were finding difficulty in putting money for emergencies. Thus, if sickness
occurred the necessary funds were just not available to receive treatment and
pay for medicines. MEDICard was born out of a desperate need.

One of the founders, who was to become the first president, Dr. Nicanor
D. Montoya M.D., held a very simple philosophy. It was that if a person could
just put away P3.00 per day, (not a large sum, even then), he could afford
hospitalization.

From this premise, MEDICard was created. An organization was formed,


MEDICard (Philippines) Inc., that would provide quality comprehensive health
care services to a large number of Filipinos, at a cost that they could afford.

Just one year later, operating from the 9th floor, Sagittarius Building, H.V.
dela Costa Street, Salcedo Village, Makati City, the company commenced
marketing the scheme. Recruitment of members was slow at first until it was

realized that more could be reached more quickly by enlisting companies who
were large employers. The advantages of this scheme to all participants were so
obvious that the marketing strategy was immediately successful.

By 1990, the success of the scheme enforced the opening of further


offices in Cagayan de Oro, Cebu and Davao City. These offices were tasked with
the coordination of accredited providers and giving assistance to members. The
expansion of the Company meant continual updating of systems and procedures.

The founders and operating officers being mainly from the medical
profession have given MEDICard a uniqueness that is difficult to be matched by
other organizations that offer medical insurance. Being of the profession, more
attention is given to the service than the finances. Investment is constantly being
made to make quality health care services available to its ever-growing
membership throughout the country. It is the dedication to good health and
services as envisioned by the founding doctors that has made MEDICard a
company whose members can always rely on.

Since its inception, the concept of service-oriented total health care has
been the molding ideal of MEDICard. The competition is vast, and the benefits
being offered by the competitors are tempting. However, MEDICard has taken
the lead in providing innovative and productive ideas that cut down the cost of
health maintenance without compromising its quality. And our predominantly
corporate- based clientele totaling about 300,000 members from more than 1700
companies can attest to the quality of service afforded to them by our network of
more than 240 hospitals and approximately 3000 doctors and dentists
nationwide.

comprehensive health care program for its members. This includes emergency
care, hospital confinement, preventive health care, dental care and out-patient
consultation services.

MEDICard is not an insurance company because unlike an insurance


company where you pay your bills for reimbursement later, MEDICard
guarantees all your medical expenses, including that all-important hospital
deposit. Then you need not worry about money. All you need is your MEDICard
ID. Today, with a total of more than 448,000 members, MEDICard Philippines,
Inc. is associated with over 8,000 accredited doctor, specialists and dentists,
operating out of over 300 hospitals and clinics nationwide. Operating 24 hours
each and every day, MEDICard Philippines, Inc. provides a quality healthcare
service to all its members.

1. Individual and Family Program

MEMBERSHIP ELIGIBILITY

Principal Member:
Any person at least 18 years old and up to 60 years old.
Qualified Dependent Member:
1. For Married Principal Members
The legal spouse up to 60 years old
Legitimate and/or legally adopted children 90 days and up to 21 years of
age who are not gainfully employed and living under the same roof as the
principal member.

2. For Single Principal Members


Parents up to age 60, unemployed and dependent on the principal
member.
Brothers and sisters 90 days old and up to age 21 who are not gainfully
employed and are living under the same roof as the principal member.

Note: The choice of enrolling dependents must follow a hierarchy. This means
that the spouse first must be enrolled followed by the eldest child, second child
and so on for principal member. For single, the parents must be enrolled first
followed by the eldest brother/sister and so on.

Deadline for enrolment of dependents:

For dependents who meet the eligibility requirements within the agreement
period -30 days form the date dependent become eligible for membership.
(copy of birth certificate or marriage contract must be submitted)

Any additional dependents other than the above can be enrolled upon the
renewal of agreement.

After the lapse of the period specified above, MEDICard will no longer
receive, evaluate and accept any designation or application to be a qualified
dependent from any PRINCIPAL MEMBER.

New enrollees who are approaching age of ineligibility must be enrolled at


least six (6) months counting from the date of effectivity up to the date that the
enrollees become ineligible for them to be accepted as members. All pre-existing
condition/ dreaded disease limits will be computed on a prorated basis (i.e. If
age of eligibility is up to 60 years old, only who are 60 years and 6 months old
and younger will be accepted for membership). Renewing members who will
become age eligible within the next renewal agreement year will be allowed to
renew regardless of the remaining months that the member will remain eligible.
However, pre-existing conditions/dreaded disease limits will be computed on a
prorated basis (i.e. If age of eligibility is up to 60 years old. A renewing member
who is 60 years and 9 months old will still be renewed but pre-existing condition/
dreaded disease limits will be computed based on the following formula: (Total
months that member remains eligible/12) x PEC/DD limit = Pro-rated Limit
In relation to his dependents, the MEMBER shall be known as the PRINCIPAL and
he shall be deemed to have undertaken to comply with all the requirements and
obligations of individual regular membership under this MMA on behalf of said
dependent/s, particularly the payment of all the required fees, dues and charges.

2. Group & Corporate Program


MEMBERSHIP ELIGIBILITY
Group Program
With 5 to 10 principal members of legal age, 18 up to 60 years old.
Corporate Program
With at least 11 principal members of legal age, 18 up to 60 years old.

3. VIP Program
EMERGENCY CARE IN NON- ACCREDITED HOSPITAL
When a member is ain immediate danger of losing a limb, eye or other
parts of the body or is in severe pain that requires immediate relief and enters a
non MEDICard accredited hospital for treatment, MEDICard agrees to reimburse
only eighty percent (80%) of the total hospital bills inclusive of professional fees
and based on relative values for MEDICard accredited hospitals, but not to
exceed the amount of P60, 000.00.

Services

1. Hospital HealthCare
MEMBERSHIP BENEFITS AND PRIVELEGES
1. The following no charge hospitalization (In-Patient) services will
apply when MEDICARD physicians prescribe the hospitalization of
members in any MEDICARD Hospitals or MEDICARD centers:

o No deposit upon admission

o Room & Board according to type of enrollment

o Use of operating theater and Recovery Room

o Services of MEDICard specialist like anaesthesiologists, internists,


surgeons, etc.

o Services and medications for general/spinal anaesthesia or other


forms of anaesthesia deemed necessary for a surgical procedure.

o Fresh whole blood transfusions and intravenous fluids

o X-ray and Laboratory examinations

o Administered medicines

o Dressings, plaster casts, sutures and other items directly related to


the medical management of the patient

o ICU confinements, Chemotherapy, Radiotherapy, and Dialysis are


covered subject to maximum limits and pre-existing conditions
coverage

o Modern therapeutic modalities and interventional surgical


procedures such as, but not limited to laparoscopic surgery and
lithotripsy, are covered up to P20,000 each per years subject to the
pre-existing conditions coverage

o CT Scan, MRI and ultrasound are covered up to P50,000 each per


member per year subject to the pre-existing conditions coverage
(inclusive of room and board, operating room charges, professional
fees and other incidental expenses relative to the procedure):

o The following complex diagnostic examinations and the therapeutic


procedures shall be covered up to P5,000.00 each per member per
year subject to the pre-existing conditions coverage (inclusive of
room and board, operating room charges , professional fees and
other incidental expenses relative to the procedure):

1. Angiography (e.g. coronary, cerebral, retinal, pulmonary, GI,


etc.)

2. Pulmonary perfusion scan

3. Tests involving use of Nuclear Technologies (e.g.


Radionuclide Ventriculography/ Thalium stress testing/
Radionucide/ Thyroid scan, etc). nuclear technologies such

as Pyrosphosphate Scintioraphy, Positron Emission


Tomography, Radio Isotope Scanning, etc.)

4. Electromyogrphy. Nerve Conduction Velocity Studies

5. 24-Hour Holter Monitoring, 2-D Echo and Doppler

6. Treadmill Stress Test

7. Myelogram

8. Endoscopy uncluding one of video

9. Orthopedic Arthroscopy

10. Adrecortical Function (e.g. Primary Aldosterinism, Cushing


Disease)

11. Plasma/Urinary Cortisol, Plasma Aldosterone, etc.

12. Mammography (breast cancer) and Sonomammogram

13. Bone densitometry scan (Dexascan)

14. Anti-nuclear antibody (ANA), C-Reactive protein (Rheumatic


and its complications), Lupus cell exam

15. Laboratory/ancillary services for conditions whose


pathogenesis or subsequent clinical improvement is not yet
fully established in Medical Science.

16. Mew modalities and/or diagnostic treatment procedures for


conditions with established etiologies and its use is only as
alternative to the conventional methods.

17. Genetic/Immunologic studies

o Assistance in administrative requirements through the liaisons


officers

o All other items related to the management of the case

Note: Above limits are inclusive of room and board, operating room charges,
professional fees and other incidental expenses relative to the procedure.

2. Non-emergency confinement or surgery (elective cases) shall be subject


to prior review and approval by the MEDICard review board. MEDICard
reserves the right to direct the members to other physicians or specialist
for further opinions as needed so as to protect the interest of both the
member and MEDICard.

3. In case a member is simultaneously covered under more than one


corporate or group health maintenance agreements with MEDICard, the
premiums for which are paid by the members employer and/or principal,
the member, on a per confinement basis, shall only avail of the benefits
accruing from one agreement. The member must choose which
agreement will apply and his confinement will be governed by the terms
and conditions and the limits of the agreement of his choice. The
provision is without prejudice to the members availment of the benefits
under another agreement which may apply for other confinements.

4. The maximum benefit limit shall be inclusive of consultations, diagnostic


procedures, and hospitalization.

5. Hospitalization or in-patient coverage of a member will depend on his/her


final diagnosis. All diagnostic procedures will depend on his/her final
diagnosis. All diagnostic procedures will only be covered if results are
within inclusions of this Agreement.

6. For confinements and availments in Makati Medical Center which has a


blanket authority policy, all disapproved charges and excess in limits shall
be settled directly with the hospital upon exhaustion of limit. Failure of

patient to settle the excess bills shall necessitate MEDICard to bill the
MEMBER with corresponding 20% service fee and payable within ten (10)
working days from receipt of billing.

7. Effective Feb. 6, 2001 Makati Medical Center (MMC) will only


accommodate HMO patients enrolled under Plan 1100 or Small Private
and up. Plans below 1100 or Ward and Semi-Private will no longer be
applicable to the said hospital.

2. Out-Patient HealthCare

MEMBERSHIP BENEFITS AND ELIGIBILITY


1. The following Out-Patient Services will be provided to members in any
MEDICARD accredited hospital:

o Referral to specialist

o Regular Consultations and treatments (except prescribed


medicines)

o Eye, Ear, Nose and Throat treatment

o X-ray and laboratory examinations prescribed by MEDICARD


physician.

2. The member can go directly to the primary physician of any accredited


hospital or at the Head Office clinic for out-patient consultation during
regular clinic hours. The primary physician will request for laboratory or
diagnostic examinations or refer the member to a specialist. The member
may avail of services from any accredited hospital of his choice.

1. When the member is under Emergency Case and ends up at the


emergency room of MEDICARD Accredited Hospitals or MEDICARD
centers, the following are provided free of charge:

o Doctors services

o Medicines used during treatment or for immediate relief

o Oxygen and intravenous fluids

o Dressings, casts, and sutures

o Laboratory, X-ray and other diagnostic examinations directly related


to the emergency management of the patient.

2. EMERGENCY CARE IN NON-MEDICARD Accredited Hospitals

1. When a member is in immediate danger of losing a limb, eye or other


parts of the body or is in severe pain that requires immediate relief
and enters a non-MEDICARD accredited hospital for treatment,
MEDICARD agrees to reimburse only (80%) of the total hospital bills
inclusive of professional fees and base on relative values for
MEDICARD accredited hospitals, but not to exceed the amount of
P5,000.00.

2. MEDICARD shall pay the said amount when it is verified that


MEDICARD facilities were not used because to have done so would
entail a delay resulting in death, serious disability or significant
jeopardy to the members condition or the choice of hospital was
beyond the control of the member or the members family. Other
expenses not covered in using non-MEDICARD Accredited Hospitals for
emergency care are:

MEMBERSHIP BENEFITS AND PRIVILEGES

The following no-charge Preventive Health Care Services will be provided


to members at MEDICard Head Office Clinic or at nay designated MEDICARD
Medical Service Units:
1. Annual Physical Examination (APE) to include:

o Complete Blood Count

o Urinalysis (urine examination)

o Fecalysis (stool examination)

o Chest X-ray

o Electrocardiogram (for adults age 40 and above, or if indicated)

o Pap Smear (for women age 40 and above, or if indicated)

2. Management of Health

3. Routine Immunization (except cost of vaccines)

4. Counselling on health habits, diets and Family Planning

5. Record Keeping of Medical History

5. Dental HealthCare

MEMBERSHIP BENEFITS AND PRIVILEGES

Members may avail of the following dental care services from any of the
accredited dental clinics:
Once a year oral prophylaxis (after having paid the annual premium)

Based to my observations when Im on the office, these are the following


findings I encountered that seem made the office engaged in the use of modern
and advance of technologies.

With e-Member Services, MEDICard members can access the following:

c. Medical and Dental Availments.
d. Request for ID Replacement.
e. Reimbursement Status/Details.
f. Medical Exam Results. (New)
g. Laboratory Exam Results. (New)
h. Diagnostic Procedure Results. (New)

2. Online Consultation:

MEDICard Philippines Inc. now offer online consultation for active MEDICard
members.

3. Search for Accredited Providers:

In this age of specialization, SEVERAL doctors are BETTER than ONE!

A. Search for Hospitals

This function will enable you to browse MEDICards accredited hospitals with
corresponding address, contact numbers, and contact person/coordinator.

B. Search for Doctors

This function will enable you to browse MEDICards accredited doctors with

This function will enable you to browse MEDICards accredited dentists with
corresponding clinic name, address, contact numbers, and clinic schedule.

4. Online Membership:

Online application or renewal of MEDICard membership. Accepts major Credit


Cards, Bancnet, and GCash.

Through the used of the cyber communication or online networking,


transaction between the company and the clients take place without any
personal dialog between the companys employees and clients. Hence, its a
great help to reduce labor cost of the company and clients time management.

ANALYSIS

Strengths of the Office:


The office offers online services like online applications or renewal of
membership, online consultation, and online search for accredited health
providers like hospitals or clinics, doctors, and dentist.
The office is equipped with CCTV cameras which are very effective in
detecting improper actions and crimes if in case, fire detectors, and fire
extinguishers.
Biometric type of log-in and log-out for employees.
Enough office supplies.
Good number of employees.

Weaknesses of the Office:


Some of the office computer units are not upgraded to the newest
technology like they still use CRT Type of monitors, DOT Matrix printers
which are very noisy, and monitors are not equipped with glass filters.
The office working area is narrow. It is like a call center booth which
employees can hardly find a space for their files and other papers.
Some air-conditions equipments are not functioning which made some
area a little hot.

WORK EXPERIENCES

First Week:

Alphabetically arranged files and removed the files of their clients that are
not updated.
Sorted files according to date, latest dates come first.
(Note): The files are owned by their clients from big companies like Meralco,
Toyota, Monde Nissin, Victoria Court, Belo Clinics, etc.

Second Week:

Groomed and punched the paper files and putted them in a folder.

Third Week:

Encoded names of companies using Microsoft Word and then I printed it.
After printing I cut the paper by company then pasted it into the folders
as label.
Kept the folders alphabetically arranged into their designated boxes in the
file room.

Fourth Week:

Encoded letters to be mailed to different companies.


Because there are some companies who did not use the name of their
mother company which is supposed to be, my trainer allowed me to
operate their system in order for me to identify the mother company of
such office.
(NOTE): My trainer holds about 100 big groups of companies or corporations.

RECOMMENDATION

A National On-the-Job training is very much different from the local one.
With all there aspects, they are different, the environment, buildings, and
especially their respective clients. I straightly recommend that National On-the-
Job training must be longer in time than local because for me getting experience
in corporate village like Makati City for only one month does not a mean of
familiarity to comply with the environments behavior and peoples activity. One
more important thing is that Metro Manila specifically Makati City is the center for
business of the country so the school should coordinated with those offices and
agencies wherein we would really see the difference between rural to urban
areas in terms of information technology which is very much significant to us as
an IT student. We should be situated in business centers where new high
technologies are seen, operated and experienced which are not really seen in the
locality.

To the office of MEDICard Philippines Inc., I recommend to widen up


working areas and set a file room for documents to be stored to avoid the over
occupancy of files in every employers booth.

CONCLUSION

Working in Makati City was really a great opportunity and experience


because it is the business center of the country. It is where you can see tall and
civilized buildings where big companies and agencies are located. For example,
MEDICard PHILIPPINES INC., which is located at the 8 th and 9th floor of The
World Center Building, is a health insurance company that holds clients from big
companies like Meralco, Victoria Court, Jaka Group of Companies, Monde Nissin,
Toyota Companies, etc.
Where in fact, my national on the-job training had thought me many things and
gave me an experience which may serve as my background on my future work
after graduation. I learned how business persons and employees dressed
themselves, how do they deal with their peers, and how do they handle
themselves and their works in their respective offices.

COMPANY PROFILE

The GSIS, created by Commonwealth Act No. 186 passed on November


14, 1936 is mandated to provide and administer the following social security
benefits for government employees: compulsory life insurance, optional life
insurance, retirement benefits, disability benefits for work-related contingencies
and death benefits.
In addition, the GSIS is entrusted with the administration of the General
Insurance Fund by virtue of R.A. 656 of the Property Insurance Law. It provides
insurance coverage to assets and properties which have government insurable
interests.

to all members and dependents, comprehensive protection to government
insurable interests and maximum contribution to nation building. We undertake

The principal benefit package of the GSIS consists of compulsory and
optional life insurance, retirement, separation and employee's compensation
benefits.

Active GSIS members are entitled to the following loan privileges: salary,
policy, emergency and housing loans.

Organization
The governing and policy-making body of the GSIS is the Board of
Trustees, the members of which are appointed by the President of the
Philippines.
The GSIS workforce consists of 3,104 employees, 52% of whom are in the Head
Office while the remaining 48% are in the Branches. To date, there are 40
branches and 78 satellite offices nationwide. It is envisioned that the System's
service network will continue to increase as the institution is committed to
provide branch offices in every province where there exists a minimum of 15,000
active members.

POLICIES AND PROCEDURES

The office is currently revising their policies and procedures so I and my co-
trainees were not given any written documentations. Beside, what I am going to
enumerate are the rules and regulations that that the Division Chief have
oriented to us during our first day of the OJT:

1. Prescribed and complete OJT uniform by the school must be wear on


Monday to Thursday while in the office.
2. Identification Card as well must be pinned while inside the office
premises.
3. Wearing of t-shirts and slippers during Friday is not allowed. Formal
business attire must be observed.
4. Observation of break time is every 10:00 to 10:15 AM and 3:00 to 3:15
PM only. Employees including trainees are not allowed to exceed within
that time. Launch break is also only on 12:00 to 1:00 PM.
5. Attendance is based on the timecard by using the Bundy clock and
timecards must not be brought home or any outside the office premises.
6. Rendering work overtime is only allowed only if with a presence of a
supervisor. Limit of work overtime is only until 7:30 in the evening.
7. Entertaining of clients and/or staying in the frontline area are not allowed
unless authorized by a superior.

DISCUSSIONS OF FINDINGS

As of to my observations, the agency do not offer life insurance benefits


only as I understand by its agency name. Through research findings, these are
the following programs and grants that the agency offers to all of its members,
specifically to all government employees:

1. Loans
Aside from the regular life insurance benefits, the GSIS also grants service
loans to its active members. These are: consolidated, policy and cash advance
loans, Bahay Ko (My House) Housing Program and Socialized/Special housing
loans. To help retirees cope with the increasing cost of living, the Pension Loan
program was conceived. The program is available to old-age or disability
pensioners under R.A. 660, P.D. 1146 or R.A. 8291.

2. Claims and Benefits


Working for the government and being a GSIS member has its rewards.
Since you are devoting so much to serve the public, GSIS is making sure that
you get all the benefits you deserve.

3. Life Insurance
As a GSIS member, automatically enjoy life insurance benefits, providing
protection for you and your loved ones. There are two kinds of compulsory GSIS
life insurance programs: The Enhanced Life Policy (ELP) and the Life Endowment
Policy.
GSIS has an Optional Life Insurance, for added coverage:

A. Enhanced Life Policy (ELP)


Enhanced Life Policy provides cover for GSIS members who entered the
service after July 31, 2003. Members whose policies have matured will also be
covered by ELP.

B. Life Endowment Policy (LEP)


Life Endowment Policy refers to the life insurance policy issued who
entered the service prior to August 1, 2003. It also provides coverage
against death whether due to natural or accidental causes, permanent
total disability and other benefits upon maturity or separation from the
service.

C. Optional Life Insurance (OLI)


Members who want to provide more protection for themselves and their
loved ones can also get additional life insurance coverage, apart from
their existing policies.

4. Retirement
The GSIS has four retirement modes available to government employees.
Those who were in the service on or before May 31, 1977 may avail of
retirement benefits under RA 660, RA 1616, PD 1146 or RA 8291. While those
who entered government service after May 31, 1977 may avail of retirement
benefits under PD 1146 or RA 8291.

* Retirement under RA 8291


This retirement mode provides two options: The Lump-sum plus Age
Pension and the Cash payment plus Pension.
A. The retiree must have rendered at least 15 years of service and must be at
least 60 years of age upon retirement.

B. He/she must not be a permanent total disability pensioner.

* Retirement under Presidential Decree 1146


The retirement mode applicable if you have been in service after May 31, 1977
but before June 24, 1997.The retiree must have rendered at least 15 years of
service, regardless of age. If retiree is below 60 years old upon retirement,
he/she may avail of retirement benefits only upon attaining his 60th birthday.
The benefit is any of the following:

* Retirement under Republic Act 660


The retirement mode applicable if you were in the service on or before May
31, 1977.
1. The retiree's last 3 years of service prior to retirement must be continuous,
except in cases of death, disability, abolition, and phase- out of position due
to reorganization.

2. His/her appointment status must be permanent in nature.

3. He/she must meet the age and service requirements under the "Magic 87"
formula, as shown below:

4. The maximum monthly pension for those above 57 years old shall be 80% of
the Average Monthly Salary (AMS) received during the last 3 years immediately
preceding retirement. The Maximum pension for those aged 57 and below shall
be 75% of the AMS
The benefit is any one of the following:
1. Automatic Pension
Retirees below 60 years old shall be qualified to a monthly annuity
guaranteed for 5 years with the option to request for a one-year

lump sum every six months. If the retiree is still living after the 5-year
guaranteed period, he/ she shall be entitled to a monthly pension for life.

2. Initial three-year lump sum


For those who are at least 60 years old but less than 63 years on date of
retirement, the benefit is a 3-year lump sum. The subsequent two -year
lump-sum shall be paid to the retiree on his 63rd birthday. If the retiree is
still living after the 5-year guaranteed period, he shall be entitled to a
monthly pension for life.

3. Five-year lump sum


Available to those who are at least 63 years of age or over on the date of
retirement. If still living after the 5-year guaranteed period, the retiree
shall be entitled to a monthly pension for life.

* Retirement under Republic Act 1616 or Gratuity Retirement Benefit


The retirement mode applicable if you were in the service on or before May 31,
1977.
1. The retiree must have rendered at least 20 years of service regardless of
age and employment status.

2. His/her last 3 years of service prior to retirement must be continuous,


except in cases of death, disability, abolition or phase out of position due
to reorganization.

5. Disability Benefits

Disability benefits are benefits granted to a member due to the loss or


reduction in earning capacity caused by a loss or impairment of the normal
functions of the employee's physical and/or mental faculties as a result of an

injury or disease. The loss in earning capacity shall be determined on the basis of
the following:

1. The employee's actual loss of income from the usual occupation


2. The capacity to engage in any other gainful occupation because of impairment

6. Unemployment

The unemployment benefit is paid when a permanent government


employee who has paid the required 12 months integrated contributions under
RA 8291 is involuntarily separated from the service as a result of the abolition of
his office or position usually resulting from reorganization. The benefit is in the
form of monthly cash payments equivalent to 50% of the average monthly
compensation (AMC). The duration of the benefit depends on the length of
service and ranges from 2 months to a maximum of 6 months.

7. Employee Compensation

The employees' compensation benefits or disability benefits is a


compensation package for public and private sector employees and their
dependents in the event of work-related injury, sickness, disability or death. The
EC is a purely employer-based contribution benefit.

8. Funeral Benefit

Beneficiaries are entitled to a P20, 000 funeral benefit payable upon the
death of a member or old age pensioner.

9. Survivorship

When a member or pensioner dies, his or her beneficiaries are entitled to


cash and/or pension benefits, Beneficiaries who qualify for survivorship pension
are entitled fifty percent (50%) of the basic monthly pension of the member or
pensioner.

10. Eligibility and benefits

Those qualified to receive survivorship benefits and the benefits they will
receive shall be the following:
(1) The survivors of deceased members who have been in the service
for 15 years or more of creditable service
(2) The survivors of deceased members who have less than 15 years of
creditable service

For those who died with the status of an inactive member and was less
than 60 years old at time of death, the cash payment/benefit shall be an amount
equivalent to 100% of the average monthly compensation for every year of
creditable service, but not less than P12,000. The deceased member must have
had at least 3 years of creditable service.

ANALYSIS

Strength of the Office


Implementation of the correct legal procedures in every transaction to be
made.
Provides frontline assistance and service to clients.
Presence of G-WAPs Kiosks in the office and other neighborhood
provinces in order for clients to check their membership status
straightforwardly.
Spacious working area for employees to work comfortable.
Proper storage of files and documents which make pull-outing easily.

Weaknesses of the Office


Some employees are not totally computer literate.
They lack familiarity on other computer applications.
The office is online connected to main office for transactions but
sometimes the network got offline which made clients transactions
extended and postponed.
Malfunctions of air some conditioners.
Lack of office supplies and equipments.

WORK EXPERIENCES

March 23 April 3, 2009


It was the first two (2) weeks of my Local OJT and as a usual deed of the
office, I and my co-trainees, both my classmates and students from University of
Saint Louis Tuguegarao, were given a short orientation and after that, we were
given a work as one. One by one, we opened old folders containing files of
pensioners (dead or still alive) searched and pulled-out for their applications,
service records, and payment vouchers. After this dusty work, we fixed or
groomed the files by cutting each edge of the papers which are extra bigger than
the other one and if the picture attached to the file is clipped by a stipple wire,
we have to detach the picture and reattached by using a paste. But its not there
that the tasks were accomplished, lastly as recommended, we have to arranged
the files alphabetically, grouped it into 30 records, and we putted it in a new
brown enveloped with number.
(Note :) Approximately there are about 700 pensioners files done so came a
long period of days.

Together with my co-trainees again and the arrived of another student
trainees from St. Paul University Philippines, CSU Aparri Campus, and CSU Carig
Campus, another type of hard work were given again to us. The mission is to
group the updates of service records and payments records of clients according
to their respective agencies or offices and after we punched the ordered papers
and filed in a thick folder in the file room.

(Note:) There are more or less 100 government offices held by GSIS Tuguegarao
Branch with many employees in each one so again we took a long period of
days.
April 13 17, 2009
I can say that the first day of this week was my independency but not. I
mean I was free from having working with my co-trainees. I was assigned in the
Claims Department that time, at the first floor of the building, and there I met
another new trainees from Cagayan State University Piat Campus. They were
three assigned in the same department and we knew each other and probably
became my friends. My first work in the department was encoding of letters to
be mailed to different offices and after was to envelop the letters. After an
inspection by Maam Eva Gammad, the Division Chief whom she was my trainer,
I bundled the letters and let it received by the secretary at the second floor. I
also had works by writing names and addresses of persons in the ready made
letters. And sometimes, some of the employee in that division let me pulled-out
files they need in the file room.

I got an alternative works, like encoded letters and put them to envelop,
encoded letter transmittals, let the letters received by the secretary, and pulled-
out files in the record room. Despites of those many works, I have managed
myself to carry them out by the helped of my friends whom I mentioned above.
There were also instances that I have done troubleshooting of computers and
printers as long as requested by the employee to do so. One good experience to
these was when the computer in the counter had no display in the monitor and
as of my inspection; I found out that it was memory loss because of it produced
a long beep upon powering it on.

It was the week for reconciliation of Liaison Officers from the different
offices to some authorized employees of GSIS so the work given to me was to
assist the assigned employee of GSIS when in case he needed files. What Ive
done if to let the liaison officers to register first through the record book, called
for their names if they were the next to face the authorized employees, and find
and gave him the files to be needed.

May 4 May 14, 2009


I can say that this last two weeks before we finally go to Manila were the
best days of my local OJT. I was finally assigned in the G-WAPS kiosks to assist
clients who are going to renew their membership, apply for a loan, and check for
their loans status. And of course, the machine used until now is a touch screen
with finger print biometrics. Whenever clients didnt know how to operate the
machine, it was my job to assist them especially those old persons who are
totally computer illiterates. Only the worst to it was the work to clean the clients
finger with alcohol in order to be recognized by the fingerprint reader. I was also
thought and instructed of what to say to clients whenever there is inconvenience
or something agency explanatory regarding such matters.

This was my comeback to the office in order to comply or complete my
hours after the National On-the-Job Training in Manila. My work was still in the
G-WAP kiosks. For the rest of the hours until I arrived at my finish time, I was
allowed to use it in gathering some important information needed on my
narrative report and my certificates as well.

RECOMMENDATION

Local on-the-job training is basically situated within the locality specifically


here in Tuguegarao City. The locality holds government agencies, private
companies or group of companies too compared in the bigger cities of the
country. As of to my observations, the group of ISAPians who previously have
their local OJT were all assigned in government offices which not all are
equipped and modernized in information technology gadgets or tools. As a
result, paper works or filing of documents was the actual scenario happened and
done everyday. My recommendation to the school is to coordinate with
companies who definitely use computers or technologies as means of working
processes or in short, IT Companies so that us students will actually experience
and explore things in relevance to our course. Thereafter, it may serve as our
very basic background for future professions and careers in life.

To the office of the GSIS Tuguegarao Branch, I recommend to increase


office supplies to the trainees in order to avoid delay of works and for trainees to
have at ease for the tasks being assined.

CONCLUSION

Working or rendering a service to GSIS Office is a great experience which


you can be proud of because as we all know, it is one of the government huge
company in which branches are located nationwide. All the supplementary
knowledge and every sole piece of ideas and thoughts gained on my local OJT
are fruits of my endurance, perseverance, industry, and focal intention to finish
my education. Despites of many trials and hard works that I experienced in every
minute during my training, I realized that OJT really did good influence and
learning for future life. Simply say without it, my future work someday is
boneless, no foundation or background at all. To be more elaborative, I know
now the basic aspects of what and how to do or what and how people do such
things inside the office.
