AFFIDAVIT OF LOSS

I, Antonio T. Gonzaga, of legal age, Filipino, married and with postal

address at Gov. Camins, Canelar, Zamboanga City, Philippines, after being
duly sworn to, in accordance with law, do hereby depose and say:

That my son Antonlou Giovannie M. Gonzaga is a

graduating student of the Asian International School of
Aeronautics and Technology, Davao City;

That as such, he was issued seven (7) solicitation tickets

to help solicit to find sponsors, patrons, benefactors and/or
advertisers;

That the tickets were sent to us to find sponsors with

solicitation ticket numbers: 0492; 0493; 0494; 0495; 0496;
0497 and 0498;

That when I checked the tickets recently, I found out that

ticket number 0496 was gone and already missing in my
possession; and that despite diligent efforts exerted to locate the
same, it proved futile and therefore considered to be
irretrievably lost;

That I am executing this affidavit for the purpose of

establishing the foregoing facts under oath for whatever legal
purpose it may serve.

address at Tugbungan, Zamboanga City, Philippines, after being duly sworn
to, in accordance with law, do hereby depose and say:

That I am a holder of voters ID duly issued by the

Commission on Elections, Zamboanga City;

That my voters ID has always been kept in my wallet for

easy access in case I need it;

That recently, I found out that my voters ID was gone

and already missing in my possession; and that despite diligent
efforts exerted to locate the same, it proved futile and therefore
considered to be irretrievably lost;

That I am executing this affidavit for the purpose of

establishing the foregoing facts under oath for whatever legal
purpose it may serve.
