Hospitals push to reduce 4.4 Million preventable hospital re-admission through the use of
different strategies and plans; such as project red, Home with made program, the transition
care modeletc. The nations health-care tab will save more than $30 billion a year or $1 of
every $10 spent on hospital care1. Louisewhose name was chosen by focus groups is
designed on how to advocate nurses interact with patients, and how to prepare them for planning
their recovery at home. This helped to cut re-admission by 30% in a 2008. Laura Landro our
author of that text-; an assistant managing editor of The Wall Street Journal, whom writes
frequently about health care; explains to us how that program is illustrated.

Summary:

Re-admission always happens because of bad instructions given to patients; In looking at

some data from the Agency for Healthcare Research and Quality, there are a large number of
customers who come back for re-admission; especially patients of heart failure, heart attack, and
pneumonia.

Study made by the Independence Blue Cross shows different conditions that may let patients
come back for re-admission; Patients may need to resume new prescription because of stopping
their medication while in a hospital, they may fail their prescriptions after they leave the hospital,
or they may just default following-up their appointments etc.2, but with new strategies;
Hospitals can avoid getting in such defaults.

Grove City Hospital in Western Pennsylvania adapts home with Meds program for helping
patients after discharge. They send them a printout of instructions and information of their
medical record and call them to discuss their medications and appointment, Gertrude Staab, 87
years old, is one of the customers who was satisfied by that program, she was admitted to the
emergency room for heart failure and discharged after 14 days, that package of medication let
her feel much easier to make all her prescriptions.

Hospitals use also Project Red program; a discharge-planning program, which developed the
virtual discharge advocate. With new procedures, nurses advocates arrange appointments,
confirm medications plans, give instructions, ask customers, and call them to identify and resolve
problems. That program is a good achievement, it helps cutting costs by $123 par patient and it
takes 30 minutes from nurses time.

 In looking at the author of the article, we see she made good job in supporting her point. She
showed us the type of researches made for the programs, and present us cases of patients. Also
the definition of terms and ideas are quoted by experienced hospitals that are working with the
programs.

Different entities of the field such as, federal medicine, government, hospitals, Louise
program, health care research, and constructors of nurses advocate are taking forward steps to
make such programs more developed, available, and useful for all hospitals.

Critique:

Our text relates to Supply chain management in a way that it delivers added service to
customers, in creating new strategies to take care of patients, and to help the healthcare center
saving. Its total quality management; which specifies task improved and staff member
initiatives. Its an involving process, where all members are united. Federal medicine,
government, hospitals, Louise program, health care research, and advocate nurses are involved
in matrix project that let patients take care of themselves after discharge.

I totally agree with the ideas of that article and the point of view of the author. Its magical to
have hospitals that use programs to take care of patients and to help their saving. I would suggest
investing that money in the development of new strategies and plans, to have good impact in
getting knowledgeable skills.
