History of Isabella
When Anna and Oswald Ottendorfer opened the first home in 1875, they named
it Isabella in honor of their daughter whose early death at the age of 27 cut
short her remarkable career in caring for the poor. Initially located in Astoria,
Queens, Isabella moved to its present location in 1889. Originally dedicated to
the care of indigent elderly women, Isabella Home quickly expanded its mission
to care for both women and men- without regard to race, creed or nationality.
In 2008, Isabella opened a new home for its expanding home care services. In
addition to the Long Term Home Health Care Program taking care of residents
in Manhattan and the Bronx, we now offer a new home care program – Isabella
Visiting Care, Inc., which expands our service area to Westchester. In
conjunction with this major expansion, we are now providing a certified Home
Health Aide Training program that enables us to train more than 100 new home
health aides a year.
Also in 2008, Isabella became the leading partner in a consortium of nine upper
Manhattan social service agencies providing care management services to
community seniors. This important consortium, called the Upper Manhattan
Partnership for Senior Independence – or UMPSI, works with more than 1200
seniors providing expanded in-home health care and meals-on-wheels.

Home Again Program
Since 1999 Isabella has been engaged in an effort to increase the comfort and
warmth that give residents the feeling of truly being "home again."
Significant steps are taken every day to enable residents to express more
control over their day-to-day living experiences. Every floor is a community
"neighborhood" where residents, family and staff get to know one another a
little bit better every-day. The Community Directors work with well-coordinated
teams of professionals in each "neighborhood" to find innovative ways to meet
the changing needs and interests of each resident.
Being "home again" is more than a concept. It's a whole new way of looking at
how we support the lives of our residents. Standardized schedules for meals
and daily activities are a thing of the past. Through extensive training, staff
members learn how to discern the interests of residents and to work
collaboratively to individualize care. Residents are now able to arrange their
schedules and activities as they wish - and not in according to a set routine.
Every day, in many ways, residents find spontaneous ways to express their
interests and personalities. Some love to gather together to share old
memories or to talk about world events. Others enjoy the companionship and
good times a songfest with music. Still others prefer the quiet refuge of a good
book in the library. The Men's Club is a big hit - and the Quilting Circle is a
popular choice for those who love to talk while being busy. Life in each
"neighborhood" can be quite bustling - and the activities vary as much as the
residents themselves.
Residents, family members, volunteers and staff play a vital role in guiding our
efforts to provide resident-centered care. Isabella's effort to create a home-like
environment is an ongoing process that welcomes new ideas from families,
staff, volunteers and of course, the residents themselves. By reaching out and
engaging all those who touch the lives of our residents, we are will continue to
fulfill our commitment to provide quality care in a home-like setting.

Palliative Care
Our “Palliative Care” Program is designed to provide “comfort and support”
when a cure is not a viable alternative. The Care Team meets with the
caregiver and resident to discuss the Program and determine the appropriate
steps to take. Together they map out a plan to provide the right kind of
comfort and support that the resident will need on an on-going basis.
The goals of palliative care are twofold: helping people suffering from illness
and pain to achieve the best quality of life possible for them; and empowering
people to become proactive in their own care and treatment.
The Principles underlying Palliative Care are:1
o Pain and illness are “whole person” experiences and have physical, emotional,
social cultural and spiritual origins and meanings to the individual. Certain
cultures are more vocal about pain; others are more stoic. Staff must be trained
to understand the meaning of illness and pain to each person and be willing to
use culturally specific treatments.
o People have different levels of tolerance for pain and different ways of expressing
it. Certain people are physiologically more sensitive to pain and easily become
anxious and upset; other people may become angry when in pain and need to
find someone to blame (often the health care provider).
o The treatment of illness and pain involves the whole team not just doctors and
nurses. Social workers, pharmacists, dietary staff, pastoral care, occupational
and physical therapists ll have meaningful roles to play in helping people achieve
an optimum quality of life
o Family’s involvement is an important part of the process. They can help staff
understand the meaning of pain to the individual and they can provide emotional
support to the patient. But families, too, need support from staff in order to
remain involved and not become exhausted and burnt out.

Isabella Nursing Home
WHAT WE PROVIDE:
Isabella is a state of the art 705 bed skilled nursing facility offering
differentiated levels of care provided in separate units:
Our focus is to provide each individual the opportunity to lead an enjoyable and
meaningful life. In addition to 24-hour medical and nursing care, we offer a
wide assortment of recreational, social and spiritual activities throughout our
facilities.
Arts and crafts, games, movies, parties and trips are just some of what you’ll
find on campus: we also provide pastoral care, therapeutic touch, exercise
programs and a lot of intergenerational fun. Our community programs offer the
same diversity of activities – carefully supervised and tailored to individual
needs.
ELIGIBILITY:
A completed Patient Review Instrument Form (PRI)
A medical evaluation completed by a physician and laboratory test
results are required if admission is from the patient’s residence

Isabella Short-Stay Rehabilitation Services
WHAT WE PROVIDE:
Isabella offers a wide spectrum of personalized rehabilitative services for those
requiring short-stay sub acute care. Our services are available seven days a
week.
OUR CARE TEAM:
Our highly skilled and experienced clinical professionals work with each resident
and his or her family to provide the guidance and care needed to ensure
maximum recovery.
Our rehabilitation staff consists of Physical Therapists, Occupational Therapists,
Speech and Language Therapists, Board Certified Physical Medicine and
Rehabilitation Physicians, Certified Therapeutic Recreation Specialists and support
staff.

Isabella Long-Term Rehabilitation Services
WHAT WE PROVIDE:
Individuals whose care needs are complex and who require a substantial period
of recovery will benefit from Isabella’s Long-term Rehabilitation Program.
Our highly trained professional staff will work to develop and supervise a
customized plan to enable each individual to regain optimal functionality.

YOUNGER ADULT CARE UNIT
Isabella has a special 33-bed unit for younger (age 20 to 60) residents who
need skilled nursing.
In addition to specialized dining services, this unit offers a wide range of special
activities and entertainment to younger residents who may require short or
long-term care. Residents are afforded a greater degree of autonomy and can
move freely about the community. They also benefit from weekly recreational
activities and social events.
RESPITE CARE
These adults can benefit from a Respite Care stay of up to 30 days in our warm,
caring environment, while the caregiver receives time to tend to his or her own
life, family or business.
Here at Isabella, individuals will have all their medical needs attended to by
Isabella’s well-trained staff, through individually-developed care plans which
may include physical therapy, speech therapy, occupational therapy and other
needed care services.
Both private pay and Medicaid participants are welcome. To qualify, the
individual must:
be age 18 or older
be under a physicians care for chronic or acute medical illness
be medically stable and free from infectious disease
provide a current medical history and results of a recent medical
exam.

Adult Day Health Care
The Adult Day Health Care Program serves adults in Upper Manhattan and the Bronx
WHAT WE PROVIDE:
We offer Personalized health care, recreation and social activities for chronically
ill adults who live at home. Care is provided according to the assessed needs of
the client.

Isabella Home Care
Offers care to those with chronic medical and mental health illness
in an individualized and compassionate manner.
WHAT WE PROVIDE:
Whether you need help caring for a loved one, or are looking for assistance for
yourself, Isabella’s caring and professional home care staff can help you
overcome your individual challenge.
Isabella Home Care offers an effective approach to care for the chronically ill
and disabled. Our services include:

Long Term Home Health Care (LTHHCP) - our "nursing home without walls" offers
care designed to assist those with chronic medical and mental health care needs who
live in Manhattan and the Bronx. Medicaid and Medicare payments are accepted.
Isabella Visiting Care, Inc. (IVCI) our Licensed Home Care Services Agency offers
flexibility and a wide array of services to meet medical, psychological, rehabilitative and
personal care needs. Services are provided by our own professionally trained and
managed Isabella staff. Residents of Manhattan, the Bronx and Westchester are
eligible. Payment by managed care and private insurance plans are accepted.
WHAT WE PROVIDE:
We offer studio and one-bedroom apartment rentals, some with balconies, all
with spectacular panoramic views overlooking New York City.
Services include:
Two meals a day - lunch and dinner.
Coffee shop and Cafeteria available for breakfast and snacks
throughout the day.
Round-the-clock security for all is provided and there is an
emergency call system in every apartment.
Complimentary weekly linen service is provided to all residents.
Housekeeping services are available at reasonable prices.
Activities, including music and exercise programs, tai-chi, pottery
and art classes, social dance, live concerts, movies, and discussion
groups.
Beauty salon, library, gift shop, laundry, and check-cashing
facilities.
Catholic, Protestant and Jewish and other
religious services are conducted.
Free visitor parking and modestly priced lodging
is available for overnight guests.
Complimentary weekly transportation is
provided to neighborhood shopping.

Isabella House apartments are spacious, sunny and
affordable. We are conveniently located on a well-tended
campus that allows residents to benefit from the full extent
of Isabella’s family of services including a medical clinic.

WHAT WE PROVIDE:
Isabella’s Child Day Care program is centered on our award winning
Intergenerational Project NOISEE: Naturally Occurring Interactions in a Shared
Environment Everyday.
The children use all of Isabella as their classroom for dynamic interactive
activities between residents of the nursing home and the children.
Open Monday to Friday from 8:00am to 6:00pm – except for
vacation periods as designated.
Specialized programs for the care of infants, toddlers and preschool
children.
Breakfast, snacks and beverages are provided. Parents provide
baby food and formula for infants and lunch for toddlers and
preschoolers.
Some of the activities we offer include:
Appropriately scheduled group activities and periods of rest
Storytelling
Music
Creative arts
Gardening
Individual lessons - which may be teacher or child initiated
Indoor and outdoor physical activities
Carefully supervised short walks to local playgrounds or parks
Computer based literacy program for children ages 2 to 5.

Early childhood education programs promote independence, self-
confidence, self-esteem and individual decision-making in children
We encourage parent involvement in all aspects of the program and
schedule parent-teacher conferences, workshops, and monthly
parent meetings.
We have a well-trained and multilingual staff and are licensed by
the New York City Department of Health.
ELIGIBILITY/PAYMENT:
Child must be 3 months old to 5 years old
Fees are structured on a weekly basis and vary depending on the
age of the child. We accept private payment and additional support
from the Agency for Children’s Services and Union Local 1199 Child
Care Fund.

Isabella Caring Partner Program
WHAT WE PROVIDE:
The Caring Partner Program trains young people to assist Isabella nursing staff
with the care of the elderly. Students are paid and work on Saturday and
Sunday from 9:00am to 1:30pm, or Monday, Wednesday and Thursday
evenings from 4 pm – 7pm.
ELIGIBILITY:
Prior experience in our community service, Summer Youth
Employment or Volunteer Programs
Age 14 or older – with working papers
Interest in working with the elderly

Home Health Aide Training Program
WHAT WE PROVIDE:
Isabella trains individuals to work as Certified Home Health Aides within a home
care setting.
The Home Health Aide Training Program at Isabella consists of more than 100
hours of classroom and practical training over a period of three weeks.
Classes are small to provide personal attention to each student's training needs.
Program content is comprehensive and includes training modules in physical
therapy, nutrition, mental health and traumatic brain injury.
Graduates of our program are free to work anywhere, although most become
employees of Isabella Visiting Care, Inc.
Eligibility Requirements:
18 Years of age or older
Successful completion of an Adult Basic Learning Exam
Must be able to read, speak and write English
Proof of ability to work in the United States
Physical exam required (we provide)

Isabella Senior Resource Center
The Isabella Senior Resource Center is an information center for seniors and for caregivers,
located in the community of Washington Heights on the corner of
169th Street and Broadway.
WHAT WE PROVIDE:
Isabella’s Senior Resource Center is located in northern Manhattan. Since it
opened in 2003, it has helped thousands of older adults and their families in the
community. Its services are offered at no cost.
ELIGIBILITY:
All are welcome although we tailor our programs towards older adults.

The Center is open Tuesdays, Wednesdays and Fridays from 9 AM – 5 PM.

Isabella Institute For Older Adults: 50 + Club
Offers health, Wellness and education programs, social events and trips for
community residents over the age of fifty.
WHAT WE PROVIDE:
The Institute for Older Adults is dedicated to providing information regarding
issues of aging, proposing innovative solutions to new challenges, and
developing effective programs to maintain individual health in later years.
Participants of the Isabella Institute have the opportunity to join the Fifty Plus
Club – which sponsors community activities to promote health and wellness.

Walking Works Wonders
A weekly exercise program led by a professional coach.
Adults of all fitness levels are welcome.
WHAT WE PROVIDE:
Walking Works Wonders is a health walking program started at Isabella more
than twelve years ago that has grown into one of the most popular programs
within the community.
Walking Works Wonders meets three mornings a week for one-hour sessions
that include warm-ups and stretching, walking, light weight training and
exercise to build up strength and flexibility. There are now more than 100
members who regularly participate in the Walking Works Wonders Program.
The program is led by a professional coach and exercise physiologist.

The fee of $1.00 a day on the days you attend

VISTAS NORC – Naturally Occurring Retirement Community
WHAT WE PROVIDE:
All residents over the age of 60 who reside at 11 and 17 Ft. George Hill or at 45
Fairview are automatically members of the VISTAS NORC and entitled to
participate in all NORC social and wellness activities. Weekly health screenings
are provided in the community room and in the privacy of your own home.
Additionally, the NORC will work with you to obtain the support services
necessary to ensure optimum quality of life if an illness prevents you from
participating in your favorite leisure activities or other activities of daily living:
washing, dressing, toileting, meal prep and eating, and walking.
Social work consultations are available to anyone living in the building that has
a need for information and referrals for themselves, a friend or loved one.

The Heart/El Corazon Neighborhood NORC
This Naturally Occurring Retirement Community brings needed services to older adults in
designated buildings
WHAT WE PROVIDE:
Isabella’s “Heart” Neighborhood NORC provides supportive services and
programs to retired adults living in the geographical area of W. 171st Street to
W.173rd Street between Ft. Washington Avenue and St. Nicholas Avenue, and
W.169th Street to W.171st Street
between St. Nicholas Avenue and
Audubon Avenue.

Upper Manhattan Partnership for Senior Independence
WHAT WE PROVIDE:
The Upper Manhattan Partnership for Senior Independence was developed as a
grass-roots partnership of leading service providers caring for seniors in this
community. Under the leadership of Isabella Geriatric Center, the consortium
provides free case management services to older adults living North of 110th
Street and West of Fifth Avenue - to the northern tip of Manhattan.

Caregiver Support Program (Co-Op)
Caregiver Ombudsman Outreach Program (Co-Op) Provides Support and assistance
for individuals coping with the demands of caregiving.
WHAT WE PROVIDE:
Isabella’s Caregiver Ombudsman Outreach Program (Co-Op) provides
assistance to individuals coping with the demands of caregiving. If you or
someone you know is in need of support and lives in Inwood, Washington
Heights, Central or West Harlem (Community Boards 9, 10 & 12), we can help
lighten the load.
The Caregiver Ombudsman Outreach Program will identify caregivers and link
them to available resources and information. To this end, we are developing
“Elderpedia”, a website that caregivers can utilize to locate community
resources.
