CURENT RATES:

Live Event/Session Work $60 - 2 Hour minimum, $25 per additional hour

Rehearsal - $20 - 1 hour minimum, $15 per additional Hour

Drumset - $30

All rates start at the time when set up begins. Rates are calculated by Dalton Hayden
Percussion Studio using an application that tracks the amount of time from unloading
until all equipment is loaded up. Time will be rounded to the nearest half hour. Time is at
the discretion of Dalton Hayden Percussion Studio.

I/we agree to release and hold harmless Dalton Hayden Percussion Studio, including all
employees from any cause of action, claims, or demands now and in the future. I/we will
not hold dalton Hayden Percussion Studio liable for any personal injury or any
personal property damage or loss, which may occur during any events which Dalton
Hayden Percussion Studio has supplied musicians for.

I/we agree to make myself/ourselves fully aware of the dalton Hayden percussion studio
policies. Furthermore, I/we agree to read and follow the policies and take full
responsibility for my/our behavior in addition to any damage I/we may cause to any
equipment owned by DaltoN Hayden Percussion Studio.

I authorize Dalton Hayden Percussion Studio to take and use photographs, video, or
other media of me/us as needed for record keeping, advertising, social media, or public
relations projects and i have have no rights to the same and will not be compensated for
the same. I understand that I may not do the same without expressed written consent from
Dalton Hayden Percussion Studio. I understand that use of photographs, video, or other
media for use of advertising, social media, or public relations projects may result in
action taken by Dalton Hayden Percussion Studio.

I understand the legal actions needed to take place for performance of copyrighted
material and release and hold harmless Dalton Hayden Percussion Studio from any legal
action taken, should my event receive any notifications from ASCAP or any other
copyright agency. I understand it is a my responsibility to complete any copyright forms
before the event, and do not put any responsibility on Dalton Hayden Percussion Studio.

My signature is proof of my intention to execute a complete and unconditional waiver and

release all of liability pursuant to the terms herein, and agreement as to all terms and
conditions above. I am of lawful age and competent to sign this affirmation.

i have fully informed myself as to the contents of this release and have read the same
prior to signing.
