Here's what to do:

1. Print yung Motion in a bond paper 8.5x13 :) pweds ka magprint sa SL's room. Please print 5 copies. Kuha ka
na rin ng Affidavit of Service Form sa OLA, kailangan din yun :)
2. Send by registered mail a copy to the adverse party. Nasa last part nung motion yung name and address
nung counsel nung petitioner. Pweds ka magsend sa UP Post Office (that's 50php - mail with registry receipt) I
will pay you tomorrow for all your expenses including transpo and food! :D hehe huhu. thank you! Tapos iattach
mo yung registry receipt na ibibigay ng post office dun sa notice and copy furnished part ng pleading (eto yung
original pleading na ififile mo in court). sa katabi nung name nung counsel for petitioner. Also, copy yung RD
number sa iba pang copies nung motion, lagay din sa katabi nung name ng counsel of petitioner.
3. Fill up the Affidavit of Service Form and then have it notarized. After notarization, attach it to the original
pleading (yung pleading na ififile mo in court).
4. Go to RTC Branch 94 of QC Hall of Justice tas file mo yung original pleading with attached affidavit of
service form. Tas pa time stamp mo rin yung ibang copies mo ng pleadings para makita na nareceive nila.
Yung ibang copies ng pleading ay for OLA.
