I, CHRISTINE MAE A. GENDRANO of legal age, Filipino, single and a resident

of Poblacion South, Polanco, Zamboanga del Norte, after having been duly sworn to in
accordance with law, depose and state:

1. THAT I am the same Christine Mae A. Gendrano, who is applying a permanent

residency with the Embassy of Canada;;
2. THAT my fathers name is Romeo Loresto Gendrano as appearing in the
certificate of his own live birth, on the other hand my very own Certificate of
Live Birth it appears that the middle name of my father which is Loristo is not
correct and misspelled because what appears in the birth certificate of my
father middle name was correctly spelled as LORESTO and not LORISTO as
appearing in my own birth certificate. NSO Birth Certificate of my father and
mine hereto attached as Annexes A and B as integral part thereof;
3. Thus, clearly therefore the difference is only with respect to the middle name
LORESTO which is the correct one because this is documented in his own birth
certificate;
4. THAT, I am executing this affidavit to attest the veracity of my foregoing
statements for whatever legal purpose it may serve.
