INSTRUCTIONS:
1. Registration is done for children under the age of 1 year only.
2. Requirements for child registration are (Please complete and check the relevant boxes):

Application Form S duly completed and signed by Father / Mother.

Copy of Child's Birth Certificate attested by Justice of Peace (JP).
Copy of Pakistani Passport and National Identity Card of Child's father attested by JP.
Copy of Pakistani Passport and National Identity Card of Child's mother attested by JP.
Three photographs of the child - Child's name written on the back.
Copy of Marriage Certificate/ Nikah nama of the parents attested by JP.
Letter of Consent from Mother / Father who is not a Pakistani citizen.
Self-addressed Express Post or Registered Post Envelop for dispatch of Birth Registration Certificate

3. For prompt service, please ensure that the above requirements are completed. Failure to provide the
required information may result in delay for which Pakistan High Commission Canberra or Consulate
General, Sydney will not take any responsibility."

Processing Fee: AU$ 20.00 (non-refundable)

Processing Time: 5 working days (only if the documents are satisfactory and genuine).

Mode of Payment:

1. In case of applying at the High Commission for Pakistan, Canberra, POSTAL MONEY ORDER /
CERTIFIED BANK CHEQUE / BANK DRAFT in favor of "High Commission for Pakistan,
Canberra".
2. In case of applying at Consulate General of Pakistan, Sydney, you can send POSTAL MONEY
ORDER / CERTIFIED BANK CHEQUE / BANK DRAFT in favor of Consulate General of
Pakistan Sydney.
3. All types of Credit / Debit Cards are also accepted for payment only in the Consulate General
Sydney through EFTPOS machine at the Consular Services Counter.
4. No Cash is accepted in any case.
