In support of this application for the issuance of a temporary

restraining order and writ of preliminary injunction, the applicant
repleads by reference all the factual allegations in the abovementioned
pleading and states:

1. The applicant is entitled to the relief demanded, and the whole or
part of such relief consists in restraining the commission or
continuation of the act or acts complained of, or in requiring the
performance of an act or acts, either for a limited period or
perpetually.

2. The commission, continuation, or non-performance of the act or

acts complained of during the litigation would probably work
injustice to the applicant.

is/are doing, threatening, or attempting to do, or procuring or

suffering to be done, some act or acts probably in violation of the
rights of the applicant respecting the subject of the action, and
tending to render the judgment ineffectual.

4. The applicant is ready and able to post a bond executed in favor of

the adverse party, in the amount fixed by the court in its order,
conditioned upon the applicants payment of all the costs which
may be adjudged to the adverse party and all the damages
sustained by reason of the injunction, if the court finds that the
applicant was not entitled to it.

5. The applicant is praying for the issuance of a temporary restraining

order and writ of preliminary injunction as the matter is of extreme
urgency and grave injustice and irreparable injury will be suffered
by the Applicant pending hearing and resolution of this application.

6. The Judicial Affidavit/s in support of this application is/are

attached as an annex/es.
