DEFENDANTS FIRST REQUEST FOR PRODUCTION OF DOCUMENTS

To: Plaintiff Justin King and his attorney, John Doe, 123 Main Street, Chicago, IL 60601.

Pursuant to Rule 34 of the Federal Rules of Civil Procedure, the Defendant requests Plaintiff to
produce and permit inspection and copying of the documents listed in this request. The
inspection and performance of related acts shall be made at a site agreed upon by the parties,
within 30 days of service of this request.

1. Produce a copy of your driving record for the previous ten (10) years.
2. Produce a copy of any medical reports and/or hospital records regarding the Plaintiffs
mental and physical health from hospitals, clinics, or other medical institutions. Include
any correspondence to and from any medical personnel, including, but not limited to
physicians, surgeons, and psychiatrists.
