IMPORTANT INFORMATION FOR APPLICANTS


1: For currently serving ADF personnel, confirmation of your aeronautical experience must be signed by your
Commanding Officer and submitted with this application. This Certification document is included in Annex 1 Section A of
this form on page 6. If your Commanding Officer is unable to confirm your experience, this can be completed by the Office
of Airlift & Training Air Force Headquarters.
2: For former members of the ADF, you must have your aeronautical experience confirmed by the Office of Airlift &
Training Air Force Headquarters. This Certification document is included in Annex 1, Section A of this form on page 6.
3: To qualify for a CASA private or commercial pilot licence, you must have completed specific courses. These courses are
identified in the certification document in Annex 1. To qualify for a flight instructor rating you must have completed the
appropriate course, identified in Annex 1, Section D iv.
4: You must include a certified true copy of specific pages of your logbook that provide evidence of your qualifications. The
pages required include the Certificate of Qualifications as a Pilot (aircraft type endorsement, initial and postgraduate
flying qualifications, competencies and renewals). You must also include the last 3 full page openings of your logbook.
5: You must meet security requirements before being issued a flight crew licence. There are a number of ways you can
meet these requirements. Refer to the ASICs and AVIDs webpage for details. Please indicate in Section A2 how you meet
the security requirements.
Note: A CASA Flight Crew Licence cannot be issued until you have been issued with an ASIC or AVID.
6: CASA must ensure an applicant is a fit and proper person before issuing a permission. Therefore the information
requested in Section A4 is still required, even if you hold a valid ASIC or AVID.
7: If you do not currently have a CASR Part 61 licence document, you may also need to submit additional documentation:
Form 61-9PIC Lodgement of Current Photo with CASA.
8: Payment for this application can be made online. Go to the CASA webpage and click the Payment button. You must
attach a copy of the receipt with this application.
9: This form can be completed electronically and saved locally to your computer. Once the ARN has been entered into the
first page, it will automatically update the applicant ARN fields on subsequent pages. CASA recommends that applicants
complete as much of the form electronically as possible.
10: If you do not have an ARN, you must submit Form 1162 (Aviation Reference Number (ARN) Application) including
appropriate identification with this application. This will satisfy the mandatory requirement for the ARN.
11: Please ensure your application and the checklist are completed correctly and that all required supporting documentation
is provided. Incomplete applications will not be accepted and may be returned to you for amendment. Fields and sections
marked with an * are mandatory.

2. Operational Ratings:
Note: For instrument and instructor ratings, your ADF qualifications must be deemed by CASA to be
equivalent to the CASR Part 61 qualification being sought. After CASA is satisfied, you will be notified in writing.
You must then complete an aeronautical knowledge examination (IREX) for an instrument rating then undertake
a flight test for issue of the rating. For an instructor rating, you must undertake a flight test for issue of the
rating. The Flight Examiner will notify CASA of the issue by submitting Form 61-2ADF.

Applicant holds, or has held a Defence Force of Australia flight engineer qualification that authorises, or authorised, the
person to perform the duties of a flight engineer without supervision in an aircraft that has a flight deck design that includes a
dedicated flight engineer duty station (it is expected an ADF FE to have passed a Basic Flight Engineer exam similar to that
of a civilian Student Flight Engineer exam as outlined in the FCL Procedures Manual can be viewed on CASA website).
