[insert name of instititution] wishes to announce its intent to form a Student Branch of the
American Society for Quality. I, a Member of ASQ, will be the Counselor for this Student
Branch and will assist the students in the process of Student Branch formation. We are
currently in the process of securing the necessary documents and completing the necessary
tasks, including sponsorship from [insert section name and number] in order to complete our
application and have it reviewed by the ASQ Board of Directors in [February], [insert year].

We would like to request your approval in our endeavor. Please provide a letter declaring your
support to ASQ Community Development at Headquarters (attached is a sample letter of your
approval), who will forward this letter on to the Chair of the Student Branch Organization and
the Chair of our Sponsoring Section.

We look forward to becoming a formally recognized ASQ Student Branch and advancing the
mission of the Society within our student community.
