I, DR. WINONA MARIE D. BAQUIAL, M.D., L.L.B, Filipino, of

legal age, single and a resident of Tagbilaran City, Bohol, Philippines
state under oath as follows:

PRELIMINARY STATEMENT

The person examining me is Atty. Kylie Jenner with office

address at 0215 CPG North Avenue, Tagbilaran City, Bohol,
Philippines. The examination is being held in his office. I am
answering his questions fully conscious that I do so under oath and
may face criminal liability for false testimony and perjury.

Q1 Do you swear to tell the truth and nothing but the truth?
A1 Yes, Maam.
Q2 Please state your name and other personal circumstances
for the record.
A2 I am Dr. Winona Marie D. Baquial, of legal age, single and a
medical examiner of Holy Name University Medical Center
Inc., and a resident of Tagbilaran City, Bohol.
Q3 How long have you been practicing your profession?
A3 For almost eight (8) years already.
Q4 Where are you presently employed?
A4 At Holy Name University Medical Center Inc.
Q5 What position do you currently hold?
A5 I am the Senior Medical Examiner in the said hospital.
Q6 Have you ever examined a certain person named Divino
Miag-ao?

A6 Yes, Maam. I have examined Divino Miag-ao after he was
rushed to the emergency room on July 4, 2017 at around
11:30.
Q7 What was your finding after conducting medical examination
of him?
A7 Upon physical examination, patient was awake, conscious,
coherent and responsive, in respiratory distress, skin is cold
to touch, clear breath sounds, tachycardic, with soft non-
tender abdomen, however, the shaft of the penis was
mutilated with lacerations and active bleeding, no scrotal
swelling, full range of motion, capillary refill time is more than
two (2) seconds and strong peripheral pulse.
Q8 Did you prepare a medical report showing such findings?
A8 Yes, Maam.
Q9 Showing to you a Medical Certificate dated July 4, 2017, is
this one.
A9 Yes, Maam. That is the one.

A10 Yes, that is my signature.

A11 No more Maam. Unless, the Honorable Court has some
clarificatory questions.
