AFFIDAVIT OF CONFORMITY

I, AILEN A. CALANTAS of legal age, Filipino, and presently residing at

Purok 1, Brgy. 2, Poblacion, Buenavista, Agusan del Norte, after being duly sworn
to in accordance with law, hereby depose and say, that:

1. I am the possessor of a certain property covered by TD No.13-01-0001-

01038, covering an area of One Hundred Twenty Nine (129) square
meters;

2. On ______________, I hired a Geodetic Engineer to conduct an actual

survey of the property as part of the requirements for the titling of the
said lot;

3. Upon actual survey of the property, the actual area size is One Hundred
Thirty Four (134) Square Meters as evidenced by the Sketch Plan hereto
attached as Annex A;

4. The boundaries stated in TD No. 13-01-0001-01038 and in the actual

survey are one and the same.

This Affidavit is being executed to attest to the foregoing facts and in

compliance with the requirements prescribed by the Department Environment and
Natural Resources.

of legal age, Filipino, and presently residing at Purok 4, Brgy. 3, Poblacion,
Buenavista, Agusan del Norte, after being duly sworn to in accordance with law,
hereby depose and say, that:

1. We are the possessor of a certain property covered by TD No. 13-01-

0001-00224 covering an area of One Hundred Twenty Five (125) square
meters and TD No. 13-01-0001-01037 covering an area of One Hundred
Twenty Eight (128) square meters respectively;

2. Based from the Tax Declaration of Real Property, the total area size of
both property is Two Hundred Fifty Three (253) square meters;

3. On ______________, we hired a Geodetic Engineer to conduct an

actual survey of the property as part of the requirements for the titling of
the said lot;

4. Upon actual survey of the property, the actual area size of both property
is Two Hundred Forty Seven (247) Square Meters as evidenced by the
Sketch Plan hereto attached as Annex A;

5. The boundaries stated in both TD No. 13-01-0001-00224 and TD No.

13-01-0001-01037 and in the actual survey are one and the same.

This Affidavit is being executed to attest to the foregoing facts and in

compliance with the requirements prescribed by the Department Environment and
Natural Resources.
