This enrollment form must be signed by a custodial parent or guardian. Signature confirms that the parent or
guardian agrees to the above financial terms and dates, and authorizes the child to attend camp. Furthermore, in
the event that your child needs immediate medical care and neither a parent nor legal guardian nor your child's
physician is available, the camp has permission to seek emergency treatment. Signature also authorizes the
camp to publish photos of the camper in marketing materials and on the camp's website. The signature on this
form also confirms that I am aware of all the trips and I agree to have my child participate. All camp paid fees
are non-refundable under any and all circumstances other than cancellation due to camper illness, which must
be verified by a physician with an official letter from the doctor. No Refunds or make up days will be granted.

with the Generations Summer Day Camp program. I agree to allow my
child to participate in all daily trips that Camp Generations will plan for
the summer. I give permission for my child to participate in all the
scheduled trips. I am aware that all scheduled trips take place within the
tri-state area. I have been given a copy of the summer camp trip
schedule for my child. I have full knowledge of the activities that have
been planned for my child on a daily basis.

I am fully aware that if I should decide not to allow my child to attend

one of the proposed trips, he/she will be required to participate in
building activities with a group that will stay in and around the camp
site. ALSO: On Big Trips, if my child is not attending child will have to
stay home for that day. All camp paid fees are non-refundable under any and
all circumstances other than cancellation due to camper illness, which must be
verified by a Physican with an official letter from the doctor . No Refunds or
make up days will be granted.

Parent /Guardian Agreement and Notice of Policies.

I hereby state that I am a custodial parent or legal guardian of the child I am enrolling and that the
child I am enrolling is healthy and fully able to participate in all camp activities. I agree to provide
Camp Generations with a properly completed medical form, based on an exam performed less than
one year of my childs last day at camp, as required by the City of New York and the Department of
Health & Mental Hygiene prior to my child attending camp.

I herby grant permission to Camp Generations to take my child on all scheduled trips and outings and
to photograph or record on video tape all camp activities and to use any photograph or video in
which my child appears in for promotional literature, displays or any other formant representing
Camp Generations to the community including the Camp Generations website. I release all rights to
such.

Payment Policy:
I agree and understand that there is a non-refundable $50 registration fee .
I understand and agree that all payments will be made in full on or prior to the payment due dates
and by the Tuesday of each week. If my payment is made after the due date I understand that the
payment must be paid by cash or money order and there will be a $25 late fee charge on all payments
made after Tuesday of each week in addition the camp balance. All camp paid fees are non-
refundable under any and all circumstances other than cancellation due to camper illness, which
must be verified by a Physican with an official letter from the doctor . No Refunds or make up days
will be granted.

Refund Policy:
I understand that no refunds or adjustments will be made for any and all absences including, but not
limited to, illness or failure to provide a medical form. Camp Generations reserves the right to
determine that you maybe eligible for a credit which can be used towards other various Camp
Generations programs ONLY. There are NO REFUNDS for any fees paid.

Bounced Check and Fee Policy:

Checks will only be taken up to June 15, 2017, after June 15, we will only take cash, money order or
credit/debit cards. I understand and agree that Camp Generations will charge me $15 plus a $25
surcharge for any bounced checks they receive from me for camp and that Camp Generations
reserves the right not to accept additional checks from me and will only accept cash, credit or money
orders for future camp payments.

T-shirt and Lunch Policy:

EACH camp day I will provide my child with his/her t-shirt and I understand and agree that if Camp Generations
has to provide my child with a camp t-shirt on any scheduled day my child must have one with him/her I agree
to be charged and pay Camp Generations ($10 for such t-shirt) . CAMP SHIRTS MUST BE WORN EVERYDAY.
Lunch will be provide FREE of charge each day by the NYC Department of Education. Daily Snacks provided by
Day Camp. Upon Entrance to Camp: I agree that my child can not enter camp until this form, application and
medical have been signed and dated and returned to the administrative office with the required payment for
entrance to camp.

**This year we are NO LONGER accepting a Supply Fee, we

would like parents to provide supplies for campers attending
camp. If you have more than one child attending, you only need
to provide supplies for one child.

Additionally: We do accept ACD & 1199, if you have ACD or 1199 please submit the forms
as early as possible so we can process your application.
