Application Form: Innovation for India Awards 2009 - 2010

Directions: Please complete this form in its entirety, and return to the address mentioned below. All information in the form will be
kept confidential and will only be used for evaluation purposes.
Qualification Criteria: To be eligible to qualify for an award:
• The applicant must have been directly involved in the breakthrough/ innovation journey/ process (may be the work of one person, a few
people or a large team or department). No third party consultants should have been involved in the process.
• Consultants may enter work they have done for clients. (In this case, they should enclose a statement from the client agreeing to be
interviewed for the in-context audit and featured in the relevant publications if the entry wins)
• The innovation will be judged against three criteria: Uniqueness, Impact and Sustainability.
• The innovation must be one that has been implemented or has yielded results within the last 5 years. All innovations that have been
implemented and demonstrated results after 2003 are eligible. Unimplemented ideas for innovation are not applicable.
• All completed application forms should be submitted by 30th September, 2009.

Organizations behind Innovation for India Awards:

Marico Innovation Foundation: Marico Innovation Foundation is a not-for-profit foundation chaired by Dr. Ramesh Mashelkar. The
foundation institutionalized the awards in 2006 and it owns the awards platform and sponsors it end to end.

Erehwon Innovation Consulting: As the architect of the awards, Erehwon conducts the necessary search and audit of all
nominations filed for the awards. In the capacity of an independent body, Erehwon’s role is to choose the top 24 candidates which
will be presented to the jury for final selection of the winners.

The Selection Process:

The Jury is a council of eminent people with deep expertise in their respective fields. They have no connection whatsoever to either
Marico Innovation Foundation or Erehwon Innovation Consulting.

The first level of shortlist will be of top 24 candidates. These will be selected by Erehwon and presented in an unbiased and equal
manner to the Jury. The Jury will then select the final list of winners who will be awarded in an honorary ceremony to be held on
12th March 2010.
