SUBJECT : GUIDELINES ON APPLICATION AND SUBMISSION OF


FORMS 48, FORM 6 AND FORM 7

1. Pursuant to CSC Rules on the filing and application of Forms 48, 6 and 7, as
reiterated during the Orientation conducted last September 8, 2017, you are hereby required
to observe the guidelines enclosed herein as agreed.

2. Failure to observe these agreed guidelines shall serve as ground for


administrative action.

GUIDELINES ON APPLICATION AND SUBMISSION OF FORM 48,


FORM 6 AND FORM 7

On Vacation Leave
1. Application for Vacation Leave (VL) shall be disapproved by the Head of Office if not
submitted at least 5 days before the scheduled leave.
2. Any absences incurred with disapproved VL shall also be automatically deducted from
the teachers salary (will be reflected in form 7).
3. Employees who are continuously absent without approved leave for at least 30 days
shall be considered AWOL (Absent Without Official Leave) and shall be separated from
the service or dropped from the rolls without prior notice.
4. Secure a copy of filed Form 6 and attach it during submission of Form 48.

On Sick Leave (SL)


1. Application of SL shall be filed immediately upon return/reinstatement to work in an
appropriate form (Form 6).
2. An application of SL which is more than five days, the employee shall be required to
attach a Medical Certificate.

On Form 6
If Form 6 are not attached during the submission of Form 48 to the Form 7 In charge,
the Form 7 In charge shall immediately return all Form 48. The School Head has to
comply and resubmit it within the day until the next day.
If the School Head does not comply within the given schedule, all teachers will be
reported as No DTR in the Form 7 and all the teachers will have no salaries including
the School Head.
If Form 6 (Application for Leave) is disapproved by the SDS for any other reasons and
the personnel is actually absent it is considered an automatic leave without pay and
corresponding deduction from salaries.
If the date indicated in Form 6 does not coincide with the absences indicated in the
Form 48, the same shall be considered as leave without pay, automatic deduction from
salary.

On Form 48
During submission - have it logged at the receiving clerk but must be handed to your
respective Form 7 In charge for further review.
Data of the print-out must coincide with the logbook in the School.
If a leave of absence is indicated in the Form 48, it must coincide with the Leave
Application Form (Form 6).
If a teacher/employee has attended Trainings/seminars or has visited the DO, a CA is
enough to support the blank date.
There should be no handwritten input of time in Form 48.
If DTRs are lacking/ data is incorrect/not submitted on or before the 5th day of the
month to the Form 7 In-charge of your school, it will be automatically returned and a
remark of NO DTR will be reflected in the Form 7, thus, no salaries for the teachers.
Claims of unpaid salaries can be possibly requested in the Division Office, provided
that you have to make the necessary steps and attach pertinent documents to support
the request for such claim.

On Form 7
Those schools who are preparing their own Form 7, deadline for submission at the DO
is every 5th day of the month.
School Heads will take the responsibility of ensuring that all necessary documents in
relation to the preparation of Form 7 is complete and correct.
Form 7 In-charge will help check and review if documents are in order.

On Salary Claims due to non-compliance of these internal policies


An Office Memo will be issued.

The teacher/school head shall submit an explanation within 72 hours.

If found justifiable, it will not form part to the 201 file.

If not justifiable, it shall form part of your 201 file.

Submit the explanation with the DTR and other pertinent documents for the processing
of salaries.
