Court ordered cannot be used

Community service logs/certificates or other proof of community service must also accompany your
reflection. Include a summary page briefly stating the following for each organization:

Reflection on Community Service


In a well written essay of two typed pages, point size 12, doubled spaced, reflect on what services
were rendered throughout the high school career, how it impacted the community, how it impacted
your life, and what impact it has had on your future. Also discuss in your reflection any positive or
negative aspects of the experience and what role (if any) the experience will play in future
community service opportunities.

LIFELONG LEARNING
One college or military visit
Students should visit at least one campus during the course of the school year, either as a school field trip or on
their own. Proof of this visit should be documented. Pictures and college info material can be used.

If a student receives an acceptance letter or acknowledgement of this application, it should be included.

Job Application and/or current Job Evaluation from current employer

Reflection on Lifelong Learning


In a well written essay of two typed pages, point size 12, doubled spaced, reflect on college
awareness, military and/or finding a job outside of high school. Students should discuss the
necessity of post high school education, steps taken to continue education and what they plan on
studying. If taking the military route, discuss the process, branch of military and what field area
they are enlisted in. If taking the work force route, how to secure employment in a career field and
area of interest.
