PURPOSE: The testimony of the Witness is offered to prove the

facts leading to the filing of this instant Petition that the birth year of
Jaymar Tocao Egkayogen was erroneously registered. Also, the
Witness will also testify on such other matters which are relevant to
this case. Further, the Witness will also identify such other
documents that are relevant to this instant Petition.

Question No. 1 (Q1): Please be informed that this examination is

under oath and that you may face criminal liability for false testimony
or perjury, should you not tell the truth. Do you understand, Madam

A: I am here to serve as a witness in support to the petition filed by

Jaymar Tocao Egkayogen ma'am.

A: Yes ma'am. I am the mother of Jaymar Tocao Egkayogen, the
herein petitioner.

Q4:Madam witness, what are the other personal circumstances that

are related to the herein petitioner?

A. Jaymar Tocao Egkayogen, my son, was born on March 28, 1996, to

me and my spouse _____ in Damawato, Datu Paglas, Maguindanao.

Q5.Do you have any proof that the birth of Jaymar Egkayogen is
registered in Local Civil Registrar?

A. Yes ma'am. The Authenticated Certificate of Live Birth issued by

the National Statistics Office ( Exhibit A')

Q6: Are the information in that Certificate of Live Birth Correct?

A: No ma'am.

Q7: what particular information in your live birth is erroneously

registered?
A: The birth year of my son, herein petitioner Jaymar Egkayogen
Ma'am

Q8: what was the year of birth erroneously registered?

A. March 28, 1992, this should be correctly be registered as March 28,
1996 ma'am.
Q9: Do you have proof that it should be March 28, 1996?
A: Yes ma'am the Affidavit of the manghihilot and Joint Affidavit of
Two Disinterested Parties (marked as Exhibits B and C)

I, Jaymar Tocao Egcayogen, single, Filipino and resident of

Damawato, Datu paglas, Maguindanao, petitioner in this case, state
under oath as follows:

Preliminary Statement:

The person examining me is Atty.______. The examination

being held at the same address. I am answering her questions fully
conscious that I do so under oath and may face criminal liability for
false testimony and perjury.

This affidavit/ testimony of petitioner Jaymar Egcayogen is

being offered to prove that the year of his birth was erroneously
registered. The petitioner's testimony is also being offered directing
the Local Civil registrar of _ to cause the correction of his erroneous
birth year.

Q. Please state your name and other personal circumstances

for the record.
A. I am Jaymar Tocao Egcayogen and a resident of Damawato,
 Datu Paglas, Maguindanao;

manghihilot and the joint affidavit of two disinterested
parties( marked as Exhibits B, C, and D)
10. Q: What are the effects of this erroneous information to you?
A: Such discrepancy greatly affects my personal data, and in
order to avoid confusion and to remedy the problem, I humbly ask
the court to direct the Local Civil Registrar to cause to be made the
correction of the my erroneous birth date, as appearing in my
Authenticated Certificate of Live Birth (Annex A) from March 28,
1996 to March 28, 1996.
