I, MELVIN RAQUINDIN LUGARES, of legal age, Filipino, and with address

at Mallig, Isabela, after having been duly sworn in accordance with the law, do hereby
depose and say that:

1. That I personally know Abel Bancud Eduarte for six (6) years;

2. That I am their farm tenant in Mallig, Isabela since 2010;

3. That I often accompany Abel Bancud Eduarte as their driver;

4. That his in laws, Urbano Salazar and Imelda Samilin Salazar are my principal
sponsors during my wedding officiated by a pastor;

5. That I am willing to donate my kidney to Abel Bancud Eduarte at my free will

and choice as well as of compassion with absolutely no monetary transaction
of any nature;

6. That Abel Eduartes mother in law, Imelda S. Salazar, will take over my
necessity in case something will happen to Mr. Abel Eduarte;

7. That I am executing this affidavit to attest the truth of the foregoing above
facts and for whatsoever legal purpose it may serve.

Gumamela St., Marulas, Valenzuela City, after having been duly sworn in accordance
with the law, do hereby depose and say that:

1. That I am suffering from Chronic Kidney Disease and was advised to undergo
kidney transplant as treatment;

2. That I personally know Melvin Raquindin Lugares for six (6) years now;

3. That he is our farm tenant and driver;

4. That upon knowing my condition, he willingly agreed to donate his kidney

without monetary transaction of any nature involved;

5. That in case something will happen to me, my mother in law will take over of
the necessities of my donor and his family;

6. That I am executing this affidavit to attest the truth of the foregoing above
facts and for whatsoever legal purpose it may serve.

2. That Melvin Lugares is willing to donate his kidney to Abel Eduarte with
absolutely no monetary transaction of any nature;

3. That I am willing to take over the necessities of Melvin Lugares in case

something happens to Abel Eduarte;

4. That I am executing this affidavit to attest the truth of the foregoing above
facts and for whatsoever legal purpose it may serve.
