(b) Eigen Real Estate Company buys cement, at a price of 500 Taka per bag, from Cemco
Cement Company for construction purpose. Annual demand of cement (at Eigen) is
73,000 bags. Eigen maintains a safety stock of 500 bags, which is 50% of average lead
time demand. Cemco requires 1 day to process a purchase order. Shipment (transit) cost,
to transport cement from Cemco to Eigen, is 10 Taka per bag. At Eigen, annual holding
cost is 20% of the value of stock. Cemco does not want to ship less than a truckload,
when a truckload is 1000 bags. Eigen's procurement lot size is two truckloads. (10+5+5)
(i) What is the shipment (or transit) time?
(ii) What is the total shipment (or transportation) cost?
(iii) What is the total annual inventory holding cost?

5. (a) A museum of natural history opened a gift shop which operates 52 weeks per year. The
authority of the museum feels managing inventories has become a problem nowadays.
Previous sales data reveals that top-selling SKU (stock keeping unit) is a bird feeder in the
museum. The following information is collected for analyzing the inventory problem:
- Sales are 18 units per week, the supplier charges $60 per unit.
- Ordering cost is $45.
- Annual holding cost is 25 percent of a feeder's value.
- Management chose a 390-unit lot size.

6. (a) A regional distributor purchases discontinued appliances from various suppliers and
then sells them on demand to retailers in the region. The distributor operates 5 days per
week, 52 weeks per year. It is mentioned that orders can be received on working days
only. Management wants to reevaluate its current inventory policy, which calls for order
quantities of 440 counter-top mixers. The following data are estimated for the mixer:
- Average daily demand = 100 mixers.
- Standard deviation of daily demand = 30 mixers.
- Lead time = 3 days.
- Holding cost = $9.40/unit/year.
- Ordering cost = $35/0rder
- Cycle-service level = 92 percent.
The distributor uses a continuous review (Q) system.
Required:
(i) What order quantity Q, and reorder point, R, should be used? (10)

(iii) If on-hand inventory is 40 units, one open order for 440 mixers is pending, and
no backorders exist, should a new order be placed? (5)
(b) Suppose, a retailer sells different types of T-shirt varying in sizes and color. The
retailer is planning to order 3000 units of a particular T-shirt for the coming Eid-AI-
Adha, 2016. It is noted that any leftover item after Eid-AI-Adha is sold at $5 per unit.
Following demand data with corresponding probability were tabulated based on previous
sales record.

7. (a) TopOil, a refiner in Indiana, serves three customers near Nashville, Tennessee, and
maintains consignment inventory (owned by TopOil) at each location. Currently, TopOil
uses TL transportation to deliver separately to each customer. Each truck costs $800 plus
$250 per stop. Thus delivering to each customer separately costs $1050 per truck. TopOil
is considering aggregating deliveries to Nashville on a single truck. Demand at the large
customer is 60 tons a year, demand at the medium customer is 24 tons per year, and
demand at small customer is 8 tons per year. Product cost for TopOil is $10,000 per ton,
and it uses a holding cost of 25 percent. Truck capacity is 12 tons.
Required:
(i) What is the annual transportation and holding cost if TopOil ships a full truckload
each time a customer is running out of stock? How many days of inventory is carried
at each customer under this policy? (6)
(ii) What is the optimal delivery policy to each customer if TopOil ships separately to

the bullwhip effect that may obstruct in implementing the coordinated supply chain. (15)

quarterly store inventory, checks employee bags before leaving, has installed security
scanners at the entrances and has put in place other measures that may affect his
company's bottom line.
(ii) Maester Aemon is a physician. He has a chamber in Oldtown. He noticed that his
medical equipment and other supplies are being stolen every now and then. He
suspected that some of his employees are behind this. By implementing new
employee oversight protocols he was able to reduce this shrinkage about 80%.

6. (a) "Industrial Hygienist requires the cunning of a detective, the discipline of a scientist,
and the persuasion of a TV evangelist" - do you agree? Justify your answer. (6)
(b) Explain with examples how a company can reduce loss exposures by means of
substitution and risk transfer. (7)
(c) Irene Adler's home which is situated at Bohemia is damaged to a large extent due to a
fire accident. She has to spend a good amount of money to restore the building, at the
same time during renovation she needs to look for an alternative residence, which would
incur her extra expense. She was also operating a business in the premises. Thus she loses
the income that used to generate from that business too. (5)
Identify the loss exposures in this example. State whether they are speculative loss
exposures or pure loss exposures.
(d) Read the following case carefully. Apply V. L. Grose's Multiple Factor Theory to
examine characteristics of each of the four Ms in this case. (17)
Myrcella, a 16 year old girl, was employed at a fast food outlet situated at Dome to cook
fries at a frying range. She slipped on water leaking from an ice-making machine and
instinctively put out her hand to break her fall. Unfortunately her hand went into the deep
fat fryer containing oil at a temperature of 3600P and she sustained serve bums to her
hand and forearm. The outlet was short staffed on the day of accident and the team leader
was working on the tills instead of monitoring workplace safety?
Although the company policy was to mop up spillages, it was common practice to leave
spillages at busy times and cover them with a sheet of cardboard, which itself can create a
tripping hazard. At busy times it was usual to give greater priority to serving customers
than to cleaning spillages.
The ice-making machine had been leaking for several days and various attempts had been
made by different contractors to cure the leak. No-one had sole responsibility to
coordinate the repair of faulty equipment and a lack of communication between different
shift managers left the equipment leaking over a long period of time.

The local authority of Dome prosecuted the company and on successful conviction the
magistrates imposed a total fine of 15000. The investigating Environmental Health
Officer believed that the accident was completely avoidable as the company had failed to
maintain a safe system of work or to carry out a suitable and sufficient assessment of the
risks associated with slipping within the kitchen.

7. (a) What type of health hazard control should be instituted first? Why is personal

protective equipment a last resort for controlling exposure? (6)


(b) List the phases of system life cycle. In which phase of system life cycle are

subsystems, assemblies, and subassemblies of the system defined? (5)


(c) Consider your Product Design II sessional project as a system. Explain the activities
you need to perform in the phase of the system life cycle mentioned in Question 7(b) to

ensure safety of the system. (6)


(d) 'Alcohol and acetaminophen' is a dangerous combination for your body. This is
because both are processed in your liver, and each puts a lot of strain on this small but
powerful organ. If you put both drugs into your body at the same time, it can overwhelm

the liver, sending it into failure. (5)


What kind of chemical interaction IS present In the combination of alcohol and
acetaminophen? Explain.
(e) The 2012 Dhaka Fire broke out on 24 November 2012, in the Tazreen Fashion
factory in the Ashulia district on the outskirts of Dhaka, Bangladesh. At least 117 people
were confirmed dead in the fire, and over 200 were injured, making it the deadliest \

factory fire in the nation's history. (6+4+3=13)


The fire, presumably caused by a short circuit, started on the ground flood of the nine-
story factory, trapping the workers on the floors above. Because of the large amount of
fabric and yam in the factory, the fire was able to quickly spread to other floors,
complicating firefighting operations. Most of the victims were found on the second floor,
where at least 69 bodies were recovered. Witnesses reported that many workers had been
unable to escape through the narrow exits of the building. The fire department's
operations manager stated that the factory lacked the adequate emergency exits that
would have made it possible to escape from the building, especially since the fire broke
out in the warehouse on the ground floor and quickly moved up to higher floors. Of the
building's three staircases, all three led through the ground floor, making them extremely
dangerous and unusable in the case of a ground floor fire. This left many workers trapped
and unable to get safety out of the course of the fire. Many of the victims were
unrecognizable because of the severity of the bums.

There are FOUR questions in this Section. Answer any THREE questions.

1. (a) A leather belt 9 mm x 250 mm is used to drive a cast iron pulley of 900 mm in

diameter at 336 r.p.m. Ifthe active are on the smaller pulley is 120 and the stress in tight
side is 2 MPa, find the power capacity of the belt. The density of leather may be taken as

980 kg/m3, and the coefficient of friction ofleatheron cast iron is 0.35. (15)
(b) A C-clamp, as shown in the Fig. 1. has trapezoidal threads of 12 mm outside diameter
and 2 mm pitch. The coefficient of friction for screw threads is 0.12 and for the collar is
0.25. The mean radius of the collar is 6 mm. If the force exerted by the operator at the

end of the handle is 80 N, find the (20)


(i) length of handle;
(ii) maximum shear stress in the body of the screw and where does this exist; and
(iii) bearing pressur~on the threads.

2. (a) An angular-contact, inner ring rotating, 02-series ball bearing is required for an
application in which the life requirement is 40 kh at 520 rev/min. The design radial load
is 725 lbf. The application factor is 1.4. The reliability goal is 0.90. Find the multiple of
rating life xD required and the catalog rating C10 with which to enter Table 1. Choose a

bearing and estimate the existing reliability in service. (20)


(b) A belt drive consists of two V-belts in parallel, on grooved pulleys of the same size.
The angle of the groove is 30. The cross-sectional area of each belt is 750 mm2 and

~ = 1.12. The density of the belt material is 1.2 Mg/m3 and the maximum safe stress in
the material is 7 MPa.

1500 r.p.m. Find also the shaft speed in r.p.m at which the power transmitted would be a

3. (a) A pair of straight teeth spur gears is to transmit 20 kW when the pinion rotates at 300
r.p.m. The velocity ratio is 1:3. The aIIowable static stresses for the pinion and gear
materials are 120 MPa and 100 MPa respectively. The pinion has 15 teeth and its face
width is 14 times the module. Determine (i) module; (ii) face width; and (iii) pitch circle
diameter of both the pinion and the gear form the standpoint of strength only, taking into

consideration the effect of the dynamic loading. (15)

(b) Design a pair helical gears for transmitting 22 kW. The speed of the driver gear is
1800 r.p.m. and that of driven gear is 600 r.p.m. The helix angle is 30 and profile is

corresponding to 20 fuII depth system. The driver gear has 24 teeth. Both the gears are
made of cast steel with aIIowable static stress as 50 MPa. Assume the face width paraIIel
to axis as 4 times the circular pitch and the overhang for each gear as 150 mm. The
aIIowable shear stress for the shaft material may be taken as 50 MPa. The form factor
may be taken as 0.154 - 0.912 IT E, where T E is the equivalent number of teeth.

4. (a) What conditions must be satisfied in order that a pair of spur gears may have a

(b) A 35 kW motor running at 1200 r.p.m. drives a compressor at 780 r.p.m. through a
90 bevel gearing arrangement. The pinion has 30 teeth. The pressure angle of teeth is

(c) Design a right angled bell crank lever. The horizontal arm is 500 mm long and a load
of 4.5 kN acts vertically downward through a pin in the forked end of this arm. At the end
of the 150 mm long arm which is perpendicular to the 500 mm long arm, a force P act at
right angles to the axis of 150 mm arm through a pin into a forked end. The lever is made
of forged steel and consists of a pin at the fulcrum. Take the following data for both the

pins and lever material: (10)


Safe stress in tension = 75 MPa
Safe stress in shear = 60 MPa
Safe bearing pressure on pins = 10 N/mm2.

5. A stock spur gear having a diametral pitch of20 teeth/in, a 0.5-in face width, 20 teeth and
a pressure angle of 200 with full depth is available. The material is AISI 1020 steel with
SuI = 55 kpsi and Sy = 30 kpsi and the safety factor is 3. What is the permissible

horsepower output ofthis gear, corresponding to a speed of 1200 rpm (35)
