7. Employment Verification:
a. Employed or Self-Employed: Notice of Tax Assessment Or Recent Payslip Or Past 6
Months of EPF contribution or Bank Statement that show multiple months of salary
b. Unemployed/ Retired: Bank Statement reflecting Name, Address, Bank Account Number
and Bank Logo
8. Futures Trading Signature Card signed and dated

Log into TDAmeritrade Asia website then follow the steps below

the 2 forms

Futures Questionnaire

And Put All No

& Continue

Futures Disclosures
Write Your Full Name and

Click I Agree
then press Save & Continue
Once done, you just need

to finish these 3 forms
For Futures Customer Agreement & Futures Addendum Form, you just need to
Write Your Full Name and Press I Agree
After that, click on Futures Signature Card and Print it out. Then fill up the
Futures Signature Card as shown below by signing on the form as shown in the
next page.
You also need to print the MAS Form 13 and fill it up as shown in the sample in
the next few pages.
After that, scan and email the form back to them. More steps in the following
pages.

How To Fund Your Account After Approval
Go to your local bank, tell them you want to do a telegraphic or Wire Transfer and say you want to
transfer money to US to buy US Stocks.
Preferred USD 3500 is for trading US Futures/Index Option (Highly Recommended amount) for
good money management.
OR Minimum USD2000 (acceptable minimum amount by broker).
Remember to include your TDAmeritrade Account Number and Your Full Name in the Wire
Transfer Form

Please ensure that you include TD Ameritrade Clearing, Inc. as the Beneficiary and the name
on your account and account number as Final Beneficiary. Because we do not accept third party
wires, the name on the wire must match the name on the account.
