PARTICULAR FORM PURPOSE


I. WORK PROGRESS REPORTS
1.0a. Monthly, 24-Hour Weather Rose To document hourly weather condition of the whole
day for the whole month

1.0b. Builders Work Progress To document the daily builders work progress,
equipment in use, personnel at the site and the field
condition on that day
1.0c. Electrical System Work Progress To document the daily electrical work progress,
equipment in use, personnel at the site and the field
condition on that day
1.0d. Sanitary/Plumbing Work Progress To document the sanitary/ plumbing work progress,
equipment in use, personnel at the site and the field
condition on that day
1.0e. Fire Protection System Work Progress To document the fire protection system work
progress, equipment in use, personnel at the site and
the field condition on that day
1.0f. Elevator Transport Work Progress To document the elevator transport progress,
equipment in use, personnel at the site and the field
condition on that day
1.0g. Air Con & Ventilation Work Progress To document daily air con and ventilation work
progress, equipment in use, personnel at the site and
the field condition on that day
2.0 Daily Construction Report To document daily work progress, equipment in use,
personnel at the site and the field condition on that
day
3.0 Monthly Manpower Loading Report To monitor and document the contractors monthly
manpower loading and development on a week basis
4.0 Monthly Equipment Loading Report To monitor and document the contractors monthly
equipment loading and deployment on a weekly
basis
5.0 Monthly Safety Statistics To document the man hours lost or affected by
accidents during a period
II. PROJECT CONTROL REPORTS
1.0 Letter of Transmittal To provide a checklist of actions on the documents
being sent
2.0 Minutes of Meeting. To document minutes of meeting
3.0 Request for Information. To document request for information
4.0 Dunning Letter To follow up pending issues
5.0 Field Order Form To be used when issuing an emergency instruction
affecting contract provision only and should be
followed with an issuance of a formal change order
6.0 Field Instruction To issue instructions or orders that are not in conflict
with the plans and specifications and without cost
implications. Otherwise, use field order forms.
7.0 Submittal Item Status To document the flow and submittal of drawings and
similar documents
8.0 Submittal Summary Status To summarize and monitor, on a periodic basis, the
various items being submitted by the contractor for
action/approval by the designers and/or the owner
9.0 Initiator Change Order Request To describe proposed changes and their justification
and to authorize issuance of a change
10.0 Change Order Authorization To provide contractual means for making changes
from original plans and specs
11.0 Change Order Summary Status To document the nature and overall cost of change
orders to the project
12.0 Budgeted vs. Committed Cost To summarize the contract cost (budgeted against
committed) per item
13.0 Project Cost Status To determine the total projected commitment and
budget cost and its variances
14.0 Contract Cost Summary To summarize the contract cost per contractor of a
particular project
15.0 Application and Certificate for Payment To facilitate the application for and payment of the
contractors works accomplished during the reported
period
III. MATERIAL REPORTS
1.0 Purchase Requisition To document the type, nature, quality and cost of
various items that need to be procured
2.0 Requisition Summary To summarize all the items requisitioned in the
project
3.0 Owner-Supplied Materials Purchased/ Documentation of all Owner-Supplied Materials
Delivery Status (OSM) Request/ Purchase Order (PO) Delivery
4.0 Owner-Supplied Materials Consumption To document consumption vs. delivery and enable

conditions during concrete batching

used
12.0 Weekly Report of Concrete Aggregates To monitor and document the quality of aggregates

13.0 Report of Compressive Tests of Concrete To regularly document the quality and strength of
concrete being used
14.0 Monthly Record of Noncomplying Tests To document the nature and number of
noncomplying testing works
15.0 Certificate of Substantial Completion Establishes a specific date that will allow project
acceptance and occupancy and final payment even
though minor tasks remain to be completed
IV. FINAL STAGE REPORTS
1.0 Affidavit of Release of Liens To submit in a sworn statement that the lien releases
or waivers attached include all parties also have lien
rights
2.0 Affidavit of Payment To document Contractors sworn statement that he
has satisfied all of his financial obligations to his
employees, suppliers, and subcontractors
3.0 Final Waiver of Lien To protect the owner from lien claims
4.0 Consent of Surety To document the consent of the Surety Company on
the Contractors Final Payment
5.0 Contractors Certificate of Completion To inform the Owner and his Engineer or Architect
that project is believed to be completed and ready
for inspections
