SUBJECT : Request of the Audit Team Leader (ATL), Audit Group C Specialty
Hospital Group, Philippine Childrens Medical center (PCMC) for
technical inspection of delivered ten (10) units Carefusion Avea Ventilator
(Pressure/Volume Ventilator) entered into by and between PCMC and RG
Meditron Inc. in the amount of Php 14,500,000.00

Please be informed that the technical review/evaluation cannot be conducted by this Office
due to incomplete submission of documentary requirements for the technical review, as follows:

1. Copy of the detailed Technical Specifications submitted/proposed by the Winning Bidder

and approved by the procuring entity;

2. Copy of Brochures/Manuals for all equipment/materials to be supplied/installed;

3. Copy of the complete sets of drawn-to-scale (not reduced size) and legible plans
submitted by the winning bidder which shall be approved by the Head of the Procuring
Entity (HOPE) or duly authorized representative; submitted plans are only preliminary
drawings not approved by the HOPE; and

4. Copy of the approved detailed breakdown of the contract cost including detailed
breakdown of estimates and/or unit price analysis for each item expressed in set and lump
sum/lot; submitted unit price analysis is incomplete and indicated several items that are
still expressed in lot and set.
