PETITION
PETITIONER, through the undersigned counsel and unto this
Honorable Court most respectfully alleges:

1. PETITIONER, __________________is a Filipino citizen, of legal age,

widow, born on __________________ and resident of

2. PETITIONER on __________________executed a LAST WILL AND

TESTAMENT in accordance with the formalities of the Republic of
the Philippines before notary public Atty. __________________and
attesting witnesses. A certified true copy of the Last Will and
Testament of __________________is hereto attached and marked as
Annex A and made an integral part hereof.

3. PETITIONER at the time of the execution of the said Last Will and
Testament was 86 years old, of sound and disposing mind and not
acting under duress, fraud or undue influence and was in every
respect capacitated to dispose of her estate by will.

8. PETITIONER moves and prays that letter testamentary be issued

required by the Rules of Court.

WHEREFORE, PETITIONER prays that after due notice and hearing

the aforementioned will of herein petitioner be approved and allowed, and
that letters testamentary be issued to the executor.
