a guide. For inter-agency,multi-level, or multi-sectoralprograms or projects, Table G.1.may be used

H. POPS Plan Monitoring and Evaluation and Reporting

Directions: In preparation for the reporting, complete the indicator profile in Table H. Using the the
same indicators in Table H, define the baseline and targets in Report Form 2. The actual
accomplishments are to be filled in for annual reporting due for submission to your local POC
Secretariat every 15th day of January of the ensuring year. For Reporting Form 1, transfer
information from Tables D and G.2 (Objectives, Indicators, PPSAs, Expected Output Targets, Target
Funding). Fill in the actual accomplishments for semi-annual reporting due for submission to your
local POC Secretariat every 15th day of July and 15th day of January of the ensuring year. If the
submission dates fall on a weekend, please submit Reporting Forms 1 and 2 on the next working
day.

to help think through the stakeholders.

Directions: Draw the communication or IEC (information, education, communication) activities


listed in the PPSAs in TableE/F and place reference numbers under C-1. You may choose to list the
activities as written in Table E/F. Considering the information provided in Table I.1., develop a
Communications Plan by completing the rest of the columns of Table I.2. Using the information in
Table I.2, draft a narrative on key communication activities, particularly explaining your approach
in meeting your communication objective.

Column 5 (C-5): Communication channel - any specific communication medium (e.g. print, video,
radio, TV, social media, personal, text blast, other electronic, or other specific information,
education, or communication activities, etc.) most appropriate for the effective delivery of your
