Script for Second Meeting of Safety and Health Team for Baraqah Engineering Sdn. Bhd.

Muhammad : Assalamualaikum and good evening everyone. I am glad to welcome all

board members to the second Monthly Meeting of the Safety and Health
Team. Thank you for coming. Can we start the meeting now?
All : Yes
Syamrie & Amirul : (talking to each other)
Muhammad : Excuse me Syamrie and Amirul, I think we already have started our
meeting can you please stop talking to each other and give me your
attention.
Syamrie & Amirul : Sorry, Mr Muhammad.
Muhammad : Its okay. So, can we start now?
Syamrie & Amirul : Ok Mr. Muhammad
Muhammad : Are there any apologies for absence for todays meeting?
Hilmi : Yes, Mr Abdullah is on medical leave.
Muhammad : Alright. Everyone, please read through the minutes of the last meeting
which were distributed earlier.

Muhammad : Are there any amendments?
Qari : Yes, Mr Chairman. There is correction to Item No. 7. Actually we agreed to take
5 more trainers from local university.
Muhammad : Thank you Mr Qari. Mr Hilmi, please look into this matter.
Hilmi : Okay, Ill make the necessary correction later.
Muhammad : Are there any other amendments?
All : No.
Muhammad : Thank you. Can someone propose the minutes to be accepted?
Amirul : I propose the minutes to be accepted.
Syamrie : I second the motion.
Muhammad : Okay, lets move on. Any matters arising?
Hilmi : Yes, Mr Chairman. There is an accident occurred in our factory on Friday last
two weeks.
Muhammad : Thank you Mr Hilmi. Can you explain to us about the accident in more detail?
Hilmi : Okay, from the report that I have received, the accident occurred in Lab 3 block
A around 3 PM. Casting machine accidently spilled during pouring molten metal
process. Luckily, no one have serious injury during the accident.

Muhammad : Do you have any suggestion, Mr Hilmi?
Hilmi : In my opinion, we should enforce a safety regulation while in laboratory. Thus,
the staff that working in lab should wear safety attire and follow all the precaution
steps to avoid any injury occurred because accident is unpredictable.
Muhammad : Thank you, Mr Hilmi. All staff please take note about this serious issue.
All : Alright/Ok/Agree.
Muhammad : Thank you Mr Hilmi, anything else?
Syamrie : Excuse me, Mr Chairman.
Muhammad : Yes, Mr Syamrie.
Syamrie : The cause of the accident have been analyzed by the technician. From the report
I received, the molten metal spilled due to broken conveyer. This is because we
bought a secondhand part of machine to reduce the cost. It has been used for
years. I think we need to replace and service all the machinery and tools in our
factory.
Muhammad : Okay, Thats good. Thank you, Mr Syamrie. Mr Hilmi, please arrange the
service schedule for all the machinery and tools. Anything else to add?
All : No.
Muhammad : Ok. I have received a report from the investigation team report about the defects
of the machine, Mr Qari can you explain more?
Qari : As you know from our investigation team report, we already going to the scene
of an accident. We had found that the conveyer of the casting machine was
broken due to the crack.
Syamrie : Excuse me, do you have evidence about that conveyer?
Muhammad : Okay Mr Qari. Do you have any evidence to support your statement?
Qari : Yes. Our inspector Mr Amirul will explain more about this.
*Amirul distribute the analysis paper to all members
Amirul : According to this report, the last inspection to the machine part was a year ago.
There was a small crack on the hanger of the conveyer but it still accepted to use
due to the NDT Safety Law, statement no. 7.
Hilmi : Sorry to interrupt, how can the machine failure?
Amirul : Thank you for the good question Mr Hilmi. The failure occur due to the over
limit use of machine. So, the only solution is by replacing the part of the machine
and doing inspection often.
Muhammad : Okay, good explanation Mr Amirul. So, any question for other member?
All : No.

Muhammad : Okay. Now lets move on to any other business. Anyone?

Qari : Yes, Mr Chairman. Just reminder to you that next month our department will
have Family Day on 20 August.
Muhammad : Thank you for remind me on that. Please remind me again the week before the
event for preparation. So, who is the Person in Charge on that day?
Qari : Its me, Mr Chairman.
Muhammad : Have you decide the venue and the activities on that day?
Qari : All the activities have been planned. But, we have two option for the venue
which is Pantai Cherating and Pantai Balok, Kuantan.
Syamrie : I think Pantai Cherating is better than Pantai Balok as it is a famous place.
Muhammad : So we have two suggestion here. Shall we put this matter to a vote? All those in
favor to choose Pantai Cherating please raise your hand.

Muhammad : Oh, majority of you are agree with this suggestion. So, its resolved that we will
have Family Day at Pantai Cherating. Okay, our next item is date, time and venue
for next meeting?
Hilmi : Im sorry Mr Chairman, since there are many activities to be completed, before
that, can I send a notice about the next meeting to the members later?
All : Yes, sure.
Muhammad : Well then, if nothing else, I think we can stop here. Thank you for coming. The
