I, X, 24 years old, single, Filipino citizen, and a resident of
CCC, after having been sworn in accordance with law, hereby depose and say:
That I am applying for issuance of a Philippine Passport with
the Department of Foreign Affairs in Cagayan de Oro City;
That in support of such application, I was required to
present certain documents as proof of my personal identity, among which are Birth Certificate and Baptismal Certificate;
That in my Birth Certificate, my family name was spelled as
AA. However, it was erroneously entered in my Baptismal Certificate as BB;
That despite this discrepancy, I hereby affirm and confirm
that the true and correct spelling of my family name is that which appears on my birth certificate which is AA, and I intend to use this spelling in my Philippine Passport for which I request issuance;
That I execute this affidavit freely and voluntarily to attest to
the foregoing facts and for the purpose of complying with the requirements of the DFA.
