Read the case notes and complete the writing task which follows.

Case Notes:

You are a pharmacist at RDBR Hospital. An elderly man, Smith Rollace,


was a type I diabetic patient. He was admitted to the hospital due to complain
related to chest pain. You are writing a letter to his wife to ensure the medication
regime is followed when he returns home.

Patient History:

Name: Smith Rollace

Date of Birth: 6 march, 1951

Allergies: Nil

Chest pain Began a day ago before admitting into the hospital on
November 12, 2010
Chest pain coincides with exertion and passes with rest
Rated pain an 8 on a scale of 1 to 10
Has had four episodes of chest pain
(Each lasted for more than a minute, generated and went off)
Last episode lasted longer than other previous episodes of
chest pain

Medical history - Diagnosed with type I diabetes at age 41


No history of any family diseases
But there is hypercholesterolemia and hypertension.

Current Medication:

Patient presents with episodes of chest pain indicated signs and symptoms of
stable angina.

An antianginal/antihypertensive agent has been prescribed: Acebutolol, 200mg,


2x/day, as well as aspirin, 150mg/day.

The patient has been prescribed Lovastatin, 40mg/day for


hypercholesterolemia

Patient has been given nitroglycerin tablets too for better relief from this angina
episode.

Other complications

The patient also shows signs and symptoms of diabetes (type I), including,
polydipsia, ketosis, polyuria, polyphagia, and dry, itchy skin.

Patient has been advised to use Alpha Keri bath oil to treat the skin.

Patient has been advised to include 30 minutes of exercise in his daily routine,
preferably during morning hours.

Drug Information: Adverse Drug Reactions

Acebutolol - Less common side-effects: changes in vision, chest pain,


difficult or labored breathing, shortness of breath, swelling of face, fingers,
feet, or lower legs, tightness in chest and wheezing.

Symptoms of overdose include: Anxiety, blurred vision, chest discomfort,


cold sweats, convulsions, cool, pale skin, cough, dizziness, faintness,
slurred speech, sudden loss of consciousness, sweating, and troubled
breathing.

Aspirin: Adverse effects include: conditions of excess stomach acid secretion,


stomach cramps, throwing up etc.

Lovastatin: Bladder pain, chest tightness, cough, difficult, burning, or painful


urination, difficulty with moving, fever, joint pain, lower back or side pain, blurred
vision, dizziness, excess air or gas in the stomach or intestines, heartburn,
indigestion, nausea.

Social History:

Patient lives with his wife, Rachael.

6:15pm: The patient was brought to the hospital in an ambulance.


Complaint of unusual chest pain while shoveling the snow, it lasted for more than
two minutes.

6:25 pm: Pt was seen by Doctor Dsouza. Acebutolol was given.


13 Nov, 2010

12 5:30pm: shifted to other ward / taken care by the doctor Andrew,


Recommended the same prescription .
Same medicine regime continued.

15 Nov 2010 Discharged from the hospital

Condition of the patient: normal


Pain: no pain (Lovastatin was recommended to gain relief from pain in future).
The patient is being discharged to the care of his wife, Rachel.

Writing task:

Using the information in the case notes, write a letter to Martha, wife of the patient,
3 Beta Avenue, Springbank, outlining her husbands medication regime, any
potential adverse effects to be aware of, and when to seek medical advice.

In your answer:

Expand the relevant notes into complete sentences

Do not use note form

Use letter format

The body of the letter should be approximately 180-200 words.
