Dear Mr. Rivas,

Our law firm was engaged by MS. SANDRA BULAK, owner of a Mitsubishi
Sedan with Plate No. ABC 1234, in connection with her demand for the payment
of damages incurred by her vehicle due to an accident involving your vehicle.

This letter pertains to the incident dated July 23, 2016 at 7:40 PM that happened
in front of Union Bank, Gen. Luna St., Iloilo City. The accident occurred when the
vehicle that Ms. Bulak was driving, traversing Gen. Luna Street was negligently
bumped by a vehicle, a LaPaz-Iloilo City route PUJ with Plate No. FGH 123 that
you were driving.

As a result of your negligent driving, damages were incurred with a grand total of
job estimate of the amount of FIFTEEN THOUSAND PESOS (P15,000.00) as
computed by Avescor Motors, Inc. Demand is hereby made for you to pay the
amount of P15,000.00 within seven (7) days from the receipt hereof.

Should you fail or refuse to pay or comply with this demand within the period
herein afforded to you, we have instructions from our client to proceed with the
possible filing of the case of reckless imprudence and damages. In addition, we
will pursue an administrative case before the Land Transportation Office (LTO)
pursuant to RA 4136 as well.

Please treat this matter with utmost importance in order to avoid a case with far-
reaching consequences.
