 Form 35 - Register of trained workers.
 Form 36 - Register of lifting fitting clothes.
 Form 37 - Register of lifting materials.
 Form 38 - Report of examination of lifts and hoists.

3) E.S.I.ACT, 1948

4) Contract Labor Act

Forms 16/17/18/20/21/22/23 shall be maintained as per the act.

• Every contractor shall be sent Half Yearly Returns to the Licensing Officer in
Form-24 on or before 31st of July.

• Every employer shall be sent the Annual Return in Form-25 on or before 15th
February of every year.

5) BONUS ACT, 1965

 Form-A - Consumption of allocable surplus.


 Form-B - Set on any set of register.
 Form-C - Totals of eligible list.
 Form-D - Annual Returns.

Annual return submitted on or before 15th December, to the Asst. Commissioner


of Labor every year.

For newly set up organization bonus will be paid after five accounting years and
calculation period from April to March.

6) GRATUITY ACT, 1972

7) Employment exchange Department :

 Form ER-II - Once in two years this shall be submitted.

8) PF ACT, 1952

Rs.6,500/- to Rs.7,500/-)

9) AP WELFARE FUND:

 Details shall be submitted once in a year @ Rs.5/- from the employee, @ Rs.15/-
from the employer deducted in the month of December. The same may be
deposited to the Welfare Fund on or before 31st of January.

OTHERS

 Monthly Electrical bills.


 Once in a year Municipal Tax.
 Water bill, phone bills, Gram Panchayat
 Plans Approvals if any.
 Plan of house keeping at work spot/sanitary/gardening etc., shall be follow up
and implemented.
