Section 1- Introduction
The BHPB Coal Exploration Incident Management Manual (IMM) has been developed and implemented to provide
a coordinated response in an emergency situation. Any person can activate this procedure at any time.

The IMM is located on IPICK.

Risk Management
The Risk Assessment process was used to identify potential emergency situations at BHPB Coal
Exploration sites and includes controls and monitoring of control outcomes to minimise risk associated
with potential emergency situations to ensure effectiveness.

Potential Emergency Situations


Potential emergency situations identified by Risk Assessment at BHPB Coal Exploration sites include:
Fires (building/equipment/bush/tyre)
Injury, illness or medical emergency
Mobile vehicle incidents (on site / off site)
Equipment structural failure (impacting on personnel or plant)
Exposure to dangerous/hazardous substances
Immersion in water
Electrocution
Natural storm events including floods
Missing persons
Mechanical energy incident including high pressure fluid release
Interaction with wildlife (livestock, snake bites).

Emergency Equipment / Facilities


Emergency equipment/facilities available in the event of an emergency include the following:

First aid supplies in all vehicles and site mobile offices


Fire extinguishers in all vehicles and site mobile offices
Remote E stops on drilling equipment
Muster points and sign in book at main entry to remote sites
Emergency Services (e.g. QAS, QRFS, and Police).

First Aid
In the event of an incident occurring at site the Supervisors are Senior First Aiders who can provide
immediate first aid to the injured person/s.
First Aid kits, located in all light vehicles, are inspected and re-stocked as required.
The Coal Exploration ERP will be activated, if required.

Risk Rating Scheme


The incident severity level is assessed using BHP Billitons risk rating scheme. Once the incident severity
has been assessed, the initial level of response required to manage the incident from both the field
response and incident management level can be determined.

Audits, Reviews and Updates


This Incident Management Manual shall be reviewed every 2 years and after any significant emergency situation
that occurs.

All emergency exercises will be audited and reviewed as part of the debrief process (Appendix 3: Incident Debrief
form)

Section 2 Crisis and Emergency Management Organisation


First Responder/s (FR)
The role of the first responder is to:

Activate the emergency response


Coordinate response actions to contain and control the scene
Provide limited primary medical care
Coordinate assistance by external agencies (e.g. emergency services)
Inform and obtain the necessary support and resources from BHPB Coal Emergency
Management Team (EMT)
Secure the scene.

Emergency Services (QAS, QRFS, Police)


The role of Emergency Services is to take necessary action in response to the incident.

BHPB Coal Support Teams


BHPB Coal support teams are fly-in teams that are on-call to provide specialist support to a site during an
emergency. These teams consist of Media Response Teams (MRTs) and Relative Response Teams
(RRTs) and their primary role is to support the FR, allowing them to focus on the immediate response to
an incident.

BHPB Coal Emergency Management Team (EMT)


The role of BHPB Coal EMT is to provide support to the FRT and manage the emergency from a business
unit perspective. The EMT will take the lead on communicating with the media and other key stakeholders.

Mutual Assistance Program


The intent of the BHPB Mutual Assistance Program is to ensure organisation-wide support for a site in the
event of an emergency. The deployment of the BHPB sites resources to the affected site is not to proceed
until directed to do so by the Vice President Planning and Technology, EMT Leader or the affected sites
FRT Leader makes a demand for support to mitigate risk of fatality or severe injury.

Section 3 Emergency Response Activation

Activation of Emergency
Due to the remoteness of site in the event of an emergency the first responder must use Mobile
Phone/Satellite phone or vehicle UHF two-way radio.

Mobile/Satellite Phone:

Dial 000 (an operator will ask what service you require Fire- Ambulance-Police)

In a slow clear voice be prepared to state:

Your name
The location of the emergency
Type of emergency injury, fire, environment
Number of people injured (if applicable)
Assistance required.
Remain on the phone ad follow all instructions given by the dispatcher.

If there is no phone service:

UHF channel 5 or 35 (this is a volunteer monitored service)


Call out Emergency, Emergency, Emergency wait up to 30sec for a response from an
Emergency Monitor before repeating

In all instances, the caller is to state:

Your name
The location of the emergency
Type of emergency injury, fire, environment
Number of people injured (if applicable)
Assistance required.
Note: Be direct in your request for assistance. Emergency monitors are not trained to give
medical advice.

Stay on channel, in case further information is required.

Immediately after activating the emergency response the first responder must contact the Manager
Exploration or the managers 1UP Head of Geoscience and Exploration

The First responder is to remain at the scene, assume the role of Onscreen Coordinator (OSC) and
coordinate actions as required.

Activation - Emergency Management Team (EMT)


If Emergency Services have been activated the Manager Exploration or, if they are the coordinating the
scene, their 1UP Supervisor, must activate the EMT by Calling the Vice President Planning and
Technology

The EMT role is to ensure more people and resources are provided to support the first responder/s who
have activated the emergency.

Aero-Medical Evacuation
All requests for evacuation by a helicopter rescue service are to be made through QAS. No calls are to be
made directly to the helicopter service. QAS will manage this evacuation from the incident site.

Section 4 Roles and Responsibilities


The EMT is activated to ensure more people and resources are provided to support the first responder/s
who have activated the emergency.

Each of the roles involved in the management of an incident shall be designated to the most appropriate
person who responds.

NOTE: Due to the limited number of personnel working on or near the work site, BHPB Coal Exploration
may engage BHPB Coal personnel external to Exploration to assist in the management of the incident.
Duty Cards
Each role has a designated Duty Card which should be adhered to in an emergency. Each Duty Card
holder shall utilise an Actions Log that shall be completed by the person responsible for that Duty Card.

Incident Management Team Leader (Duty Card 1)


(Manager Exploration or, if they are first responder, their 1 Up)
The Emergency Management Team Leader shall immediately communicate directly with the OSC on all
aspects of the emergency situation and review the current status of the emergency. This may include:
Confirm nature and level of alert of emergency
Delegate responsibilities as per duty cards
Liaising with stakeholders (ERT/Emergency Services/Work Place Health and Safety)
Ensuring the site is secure

On Scene Coordinator (Duty Card 2) (Supervisor & Superintendent)


The role of the OSC is to coordinate the response at the scene and ensure that they are provided with the
support required to resolve the emergency. This may include:

Liaising with emergency services agencies at the incident scene


Requesting and coordinating additional people and resources
Keeping the EMT Leader informed of the situation

Escort Vehicle (Duty Card 3)


The role of the Escort Vehicle is to provide assistance in directing external services to the scene of the
incident. This may include:
Unlocking relevant gates
Meeting Emergency Services on primary roads

Emergency Desk (Duty Card 4)


The role of the Emergency Desk is to attend the Central Queensland Office as soon as possible after
activation, record all actions, response and other critical information and decisions in the Log.
Communications / liaise with BHPB Coal Communications / External Affairs representative. This may
include:
Making/answering calls and relaying messages as requested
Assisting in administrative and support duties
Being first point of contact for people on site
Provide regular updated information to the BHPB Coal Communications / External Affairs
representative
Deliver stakeholder communications provided by BHPB Coal Communications / External Affairs
representative to media, employees/contractors and employees friends and families

First Response Team Member (Duty Card 5)


The role of the First Response Team Member is to attend and assist at the scene of an emergency
when the emergency procedure has been activated.
This may include:
Completing risk assessments to ensure the safety and security of people and assets
Attending to injured personnel within their level of competency
Make the area secure and safe without compromising safety and health standards
Assist the First Responder/OSC and Emergency Services as required

Section 5 Incident Management Room


In the event that an Incident Management Room is required to manage the emergency effectively, utilise
the Central Queensland Office.

Section 6 Communication
Communication Response
When an emergency occurs, the sites Emergency Desk (Duty card 4) must advise the BHPB Coal
Community/ External Affairs Manager.

BHPB Coal is responsible for managing all stakeholder, media enquiries and releases; however, the EMT
and Emergency Desk must be prepared to manage media that arrive on site. In the event that media is
expected to arrive on site Emergency Services (Police) will be called to manage the situation.

Managing Media / Government / General Enquiries


Inform the person that you are not an authorised spokesperson, and as such you are not in a
position to comment or provide detailed information. Transfer the call immediately to the
Communities / External Affairs officer or take their name and contact number and organise for
them to be called back
Log the enquiry on the Individual log sheet.

Next-of-kin Notification and Management


Notifying next-of-kin of an injury
If a serious injury occurs to an employee as a result of a work related incident, the employees next-of-kin
and other family members require prompt, specific and empathetic assistance.

If the members next-of-kin live local to the site of the injury, the senior manager on site and the victims
immediate supervisor should notify the next-of-kin in a face-to-face visit. In the event that families of
impacted employees are not local to the mine site where the incident occurred, BHPB Coal will co-ordinate
notification through other BHP Billiton Assets in the familys location.
