Notes: Goal 1: Anna made lots of eye contact and was smiling. We began by started with a verbal cue by asking her to only push the right button as the left didnt have stimuli. In
the second trial for this goal during the 2nd time she pushed both buttons and we prompted her by saying, only one at a time Anna. During the 3rd trial for this goal we started
with a partial physical prompt to remind Anna where the squeaky toy was since we had switched hands again. On the 5 th try during this trial Anna pushed the squeaky toy with her
left hand (midline). Goal 2: There were many extra hits in between turns each trial. Anna was very wiggly. More grunting during without sound trial. Used forearm during 3rd trial
on the 5th turn. Not a lot of eye contact. Not sure how purposeful her movements were. Goal 3: Used lateral arm movement, all right arm. Started with 2 full assists to introduce
activity as this was a new arm movement and we wanted Anna to know what to do. Here it is Anna verbal cue from mom. During the 8th turn Anna used a flat hand on the switch
and pushed forward. If arm was locked during this activity she would move arm around and then attempt the lateral movement again. Overall: We need to extend difficulty.

Impression:
Anna seemed to enjoy our activities we had planned for her. She was happy and breezed through the activities very quickly. Anna was also very
wiggly tonight so we werent quite sure how purposeful her movements were.

Objective Description of Data and Observations:


First trial was about making choices. The goal is that Given trunk support and options Anna Carter will demonstrate her cognitive ability to show
preference for one of two options, using buttons. During this first trial we gave Anna the options of her squeaky toy or nothing. We slid each
button down one of her arms to let her know where they would be and gave her the corresponding stimuli. After this initial prompting we did not
have to prompt her again for this activity.
Second trial was about distinguishing whether or not Anna would pay attention to an activity more with or without sound. We showed her the IPad
and then slid the Bluetooth button down her arm so she would know where it was. We switched every 5 trials alternating having sound and
without sound. For this trial no other prompting was needed. Anna was very wiggly tonight; thus it was not possible to know how purposeful her
actions were.
The third trial was all about having Anna control the stimuli she received, as well as when she stopped receiving it. We used a latch switch device
where she would push when she wanted it on and push when she wanted it off. Some verbal prompting was needed for this trial as well in the
form of family or us saying, will you turn the lights on Anna? or Will you turn the lights off Anna? Anna showed the cognitive ability to
respond to these prompts by completing the action necessary to receive the stimuli.

Assessment and Evaluation:


This data is showing that Anna is getting these activities. For the most part she had independent success 100% of the time, the times that she
didnt have success, she still made an attempt at completing the activity. During all three trials Anna never needed more than a verbal prompting. I
think this means we need to increase the difficulty of some of the trials. Based on observation and data we cant be 100% sure of the purpose
behind Annas actions because she seemed very active in regards to extra movements tonight. Based on data we can see that Anna has the
cognitive ability to understand when we ask her to complete a specific action in regards to her goal, ex. When she was prompted with the phrase
Will you turn the lights off Anna? she complied with the request. Based on observation and conversation with mom and during a team
discussion I can say our performance was good. We allowed mom to complete the prompts so that they can practice these goals without us. We
also were able to discuss the changes we may want to make in regards to extending these activities. Based on the data collected, we think it is time
to increase the difficulty of our activities given her independent success.
Plan:
Our plan currently is to stick with our activities, changing some of the stimuli, for example using a squeaky toy or tambourine instead of water
marbles and using a fan instead of lights. Some ideas we had for extension for goal 1 include: try the activity with Anna in her chair instead of in
her moms lap. Have three choices instead of two, using Annas feet. Try encouraging more midline work. For goal 2: Have screen closer to her so
we can observe more eye contact. Goal 3: Try different arm movements. We will continue to offer verbal prompting as needed. Annas goals
given her IEP are; Given trunk support and options Anna Carter will demonstrate her cognitive ability to show preference in daily
routines 8 out of 10 times without teacher prompting by 10/24/18, Given trunk support Anna Carter currently demonstrates cognitive ability to
control when she receives visual and auditory stimuli from the IPad, by pressing the button on her lap, 3 out of 10 times without teacher prompt,
and Given trunk support Anna Carter currently demonstrates the cognitive ability to control when she receives any
stimuli that is connected to a switch device 6 out of 10 times without teacher prompts.
