AFFIDAVIT OF EXPLANATION
I, Atty. Jeremias C. Garcia, of legal age, married, Filipino and a resident of
#171 Lourdes, Sta. Rosa, Nueva Ecija, after having been duly sworn to in
accordance with law, depose and say;

1. That I am a Notary Public commissioned for the City of Cabanatuan,

Municipalities of Aliaga, Llanera, Gen. Natividad, Rizal, Sta. Rosa and
Zaragoza, Nueva Ecija for the term January 14, 2015 to December 31, 2016
with office address at 456 Ortiz Apartment, Maharlika Highway, Dicarma,
Cabanatuan City, Nueva Ecija;

2. That in my notarial report for the month of May 2016, the copy of a certain
document identified as Doc. No. 213 in Page No. 44 of Book No. 51 Series of
2016; July 2016, identified as Doc. No. 295 in Page No. 60 of Book No. 52
Series of 2016; and for the month of September 2016, copies of certain
documents identified as Doc. No. 175 in Page No. 36, Doc. No. 208 in Page
No. 43, Doc. No. 296, 297, 298 and 299 in Page No. 61 of Book No. 53 Series of
2016 have not been included in the bounded files submitted to the Office of
the Executive Judge, Notarial Section thru the Clerk of Court because my
legal staff in the office failed to notice the deficiency of documents
submitted thereon;

3. That I am executing this affidavit of explanation in good faith and not for any
other unlawful purpose/s and to attest to all the facts above stated;

AFFIDAVIT OF EXPLANATION
I, Atty. Jeremias C. Garcia, of legal age, married, Filipino and a resident of #149 Louders, Sta.
Rosa, Nueva Ecija, after having been duly sworn to in accordance with law, depose and say;

1. That I was a Notary Public for the City of Cabanatuan, and for the Municipalities of Aliaga,
Llanera, Gen. Natividad, Rizal, Sta. Rosa, and Zaragosa, Nueva Ecija, for the term January 4,
2013, up to December 31, 2014 with office address at 408-C Cerin Bldg., Maharlika Hi-way,
Brgy. Dicarma, Cabanatuan City, 3100;
2. That in the following notarial reports for those particular months, the following were
inadvertently not signed by the notary public:

2.1 January, 2013, Document Nos. 28 and 75 in Book 36;

2.2 October, 2013 Document Nos. 266 and 353 in Book 38;
2.3 December, 2013 Document Nos. 139 and 143 in Book 39; and
2.4 February, 2014 Document Nos. 363 and 365 in Book 40.

3. That in the following notarial reports for those particular months, the following were
inadvertently not included :

3.1 January, 2013 Document Nos. 119 and 124 in Book 36;
3.2 September, 2013 Document No. 215 in Book 38;
3.3 April, 2014 Document No. 245 in Book 41. and
3.4 July, 2014, Document No. 365 in Book 42;

4. That in the following notarial reports for those particular months, only photocopies of the
following were inadvertently submitted;

4.1 July, 2013 Document No. 381 in Book 37; and

4.1 January, 2014 Document No. 129 in Book 40.

5. That in the notarial report for this particular month, the following were inadvertently doubly
entered ;

5.1 May, 2014 Document Nos. 392 and 293.

6. That, in the month of March, 2014 two documents entered/submitted/filed thereon as

Document Nos. 57 and 58 in Book 41, the handwriting looked like 58, but it should be 57
hence, there was no double entry entered/submitted;

7. That the late submission of at least two (2) notarial reports in 2013 due to the heavy clerical
work that my secretary attended to during those months, he was not able to complete and
submit our monthly notarial report on time;

8. That I am executing this affidavit of explanation in good faith and not for any other unlawful
purpose/s and to attest to all the facts above stated;

AFFIDAVIT OF EXPLANATION
I, Atty. Jeremias C. Garcia, of legal age, married, Filipino and a resident of Sta. Rosa,
Nueva Ecija, after having been duly sworn to in accordance with law, depose and say;

1. That I am a Notary Public for the City of Cabanatuan, and for the Municipalities of Aliaga,
Llanera, Gen. Natividad, Rizal, Sta. Rosa, and Zaragosa, Nueva Ecija, for the term January 6,
2011, up to December 31, 2012 ;
2. That in my notarial report for the month of September, 2011, a certain document
entered/filed/submitted thereon as Document No. 80 had no signature/s of the party/parties thereof
because because my legal staff in our office has inadvertently overlooked the same ;

3. That in my notarial report for the month of February, 2012, a certain document with Document
No. 243 was not included in the file thereof and also a certain Document with Document No. 287
was not signed by the herein affiant/Notary Public because the same were inadvertently overlooked
by my legal staff, likewise, in the succeeding month of June, 2012, a certain document entered
thereon as Document No. 239 was only a xerox copy of the original, while in the following month of
July, 2012, certain documents entered thereon as Document No.s 281, 282, and 283, were doubly
entered thereon because my legal staff in our office has also inadvertently overlooked the same ;

4. That likewise in the above-said month of July, 2012, Document No. 355 was seemingly entered
thereon without the signature of the parties/signatories thereof because their signatures were in the
separate sheet/page thereof ;

5. That in the in the succeeding/following months of August and September, 2012, certain
document entered/submitted/filed thereon as Document Nos. 107 and 442 had no signatures of the
signatories thereof because the same were also inadvertently overlooked by the legal staff of our
office, while Document No. 477 was seemingly entered thereon without the signatures of the
parties/signatories thereof because their signatures were in the separate sheet/page thereof ;

6. That the late submission of the notarial report for the months of January, March, May, June, July,
August, September, October of 2012 and November, 2010 was due to the heavy clerical work that
my secretary attended to during those months that he was not able to complete and submit our
monthly notarial report on time the months aforementioned ;

7. That I am executing this affidavit of explanation in good faith and not for any other
unlawful purpose/s and to attest to all the facts above stated;
