Psychosexual disorder

- In ability to become sexually aroused or achieve sexual satisfaction in the appropriate


situations due to psychological reason.
- Sexual problem that is psychological in origin

Transsexual

- a person who undergone a sex change operation

Sexual heath

- state of physical emotional and mental or social well-being in relation to sexuality

Sexual desire

- a subjective feeling state and an interest in sexual

Transgendered

- gender expression behavior not conform to that typically associated with the sex to
which they were assigned at birth

Erogenous zone

- an area of the human body that has heightened sensitivity the stimulation which
may result in the production of sexual fantasies sexual arousal and orgasm

Gender dysphasia

- Condition characterized by a disconnection between someones assigned and perceive


genders.

Sexual orientation

- sexual attraction to men, women, gender

Paraphilia

- describes arousal to objects, situation or individual that are not part of normative
behavior
Psychosexual Dysfunction:

General

Etiology:

1. psychological conditions
- depression
- anxiety
- guilty feelings
- stress
- negative body image
- traumatic sexual experience
- relationship problems between the two people
2. physical conditions
- endocrine illnesses
- neurological conditions
- gynecological problems
- cardiovascular problem
- respiratory problems
- renal problems
- drug problems
- anti- inflammatory hormone

Diagnostic:

a. symptoms manifested
b. medical history and medications currently taking
c. sexual history
d. physical exam
e. examination of the partner
f. tests:
blood tests
depression scale
mini mental state examination
vascular assessment
gynecologic examination
vaginal samples
Symptoms for men:

- not able to keep an erection


- premature ejaculation
- ejaculation do not occur
- not able to become aroused when appropriately stimulated
- not able to achieve orgasm
- inhibited sexual desire

Symptoms for women:

- inhibited sexual desire


- not able to achieve orgasm
- dry vagina
- vaginismus
- experience pain during intercourse

Treatment options

a. medications
phosphodiesterase inhibitors- During an erection, blood fills tissue in the penis,
causing it to enlarge and become stiff. The PDE-5 inhibitors relax smooth muscle,
allowing the penis to fill with blood.
vasoactive agents- cause blood vessels of penis to expand thus increasing blood flow
and resulting in an erection
testosterone replacement therapy
mechanical devices to maintain an erection
anesthetic creams, vaginal estrogen creams or rings, lubricants

SEXUAL DYSFUNCTION
- Disturbance in the sexual response cycle as pain with sexual intercourse

a. SEXUAL AROUSAL DISORDER


- Frigidity to women and impotence to men
- An aberration during any stage of sexual response that prevents the experience of
satisfaction through sexual activity
Female sexual disorder
- Characterize by persistent or recurrent period or complete failure to attain or
maintain the lubrication swelling response of sexual excitements until the
completion of sexual activity

Clinical manifestation:
Inability to relax vaginal muscles enough to allow intercourse
Inadequate vaginal lubrication before and during intercourse
Inability to attain female orgasm

Male erectile disorder


- impotence or erectile dysfunction
- characterized by recurrent and persistent partial or complete failure to obtain
or maintain an erection to perform the sexual activity

2 types of erectile dysfunction


1. primary
never had erection
2. secondary
less erection

Clinical manifestation:

- Inability to achieve erection


- Inability to maintain adequate erection for intercourse
- Delay or absence of ejaculation despite adequate stimulation
- Inability to control timing of ejaculation

COMMON CAUSE OF IMPOTENCE:


- Medications use like antihypertensive drugs
- Smoking
- Hypertension
- Hormonal deficiency caused by disease
- Urological procedure
- Depression, anxiety, fatigue, boredom, stress
- Penile implants
- Mood altering drugs
b. SEXUAL DESIRE DISORDER
- Decreased libido
- May result from decreased in androgen and testosterone hormone production
Causes:
- Aging
- Fatigue
- Medication
- Depression
- Anxiety
- Pregnancy

c. SEXUAL ORGASMIC DISORDER


- A persistent delay or absence of orgasm following sexual excitement
- Inability to achieve climax during sexual stimulation
- Often occurs in conjunction with other sexual dysfunction
- Primary or lifelong disorder

FEMALE ORGASMIC DISORDER


- Inhibited female orgasm, anorgasmia
Causes:
Spinal cord lesion
Medication side- effects
Post- sexual abuse
Removal of clitoris
Emotional abuse
Illicit substance abuse
Fear of becoming pregnant
Fear of rejection of partner
Self- image problems
Relationship problems
Major depression

MALE ORGASMIC DISORDER


- Inhibited orgasm or retarded ejaculation

Causes:
Thyroid disorder
Pituitary condition
Disease of the penis
Spinal cord injury
Substance abuse including alcohol
Medications like thorazine, aldactone, propranolol, viractil
Depression
Feelings of guilt, anger, fear, low self- esteem, anxiety
Fear of getting the partner pregnant or contracting a sexually
transmitted disease
Environmental factors
Past history of traumatic sexual encounter

FOUR MAIN EJACULATORY DISORDERS


1. Retrograde
Semen is passed in a retrograde fushion into the bladder as opposed to
out the urethra
2. Premature
Rapid ejaculation
3. Retarded orgasm
Inability to achieve orgasm on any occasion
4. Anejaculation
Pathological inability to ejaclutae in males orgasms or anorgasmic

TYPES OF FEMALE OR MALE ORGASMIC DISORDER:


1. Primary
- Or a lifelong; never experienced orgasm through any means of stimulation
2. Secondary
- Or acquired; often temporary, woman had attained orgasm in the past but is
currently non- orgasmic.
3. General
- Failure to have orgasm occurs with different partners and in many different
settings
4. Specific/ situational
- Occurs only with specific partners or under particular circumstance

d. SEXUAL PAIN DISORDER


1. Dyspareunia
- Recurrent genital pain occurring in either men/ women during/ after
intercourse
2. Vaginismus
- Involuntary spasm of the musculature surrounding the vagina causing it to
close, resulting in penetration being difficult and painful
- TYPES:
Primary- never at anytime been able to have pain free intercourse
Secondary- develops later in life even after many years of pleasurable
intercourse
- Causes:
Past sexual trauma abuse
Psychological factor
History of discomfort with sexual intercourse
Unknown
- Manifestations:
Vaginal pain during sexual intercourse
Painful, difficult or impossible vaginal penetration during sex

Physical exam
- Treatment
Education
Counseling
Kegels exercise
Vaginal dilation exercises

e. SUBSTANCE- INDUCED SEXUAL DISORDER


- Sexual dysfunction due to a prescription drug or drug abuse of a substance that a
person is taking
- Manifestation:
Impaired arousal
Sexual pain
Impaired sexual desire
- Treatment:
Stop the medication

f. PARAPHILIA
- Paraphilias are persons who have not yet reached sexual maturity
- Involves a typical or extreme sexual behavior

Characteristics of paraphilia

- Unusual or bizarre behavior is required for sexual stimulation and gratification


- Excessive amount of imagination must be present
- Preference for non- human objective for sexual stimulation
- Preference for sexual activities with partners who dont give consent
- Preference for sexual intercourse with minors

PARAPHILIC ACTIVITIES

Exhibitionism

- Recurrent sexual urges exposing ones genitals to unsuspecting strangers

Fetishism

- Sexual urges or behaviors using nonliving objects

Frotteurism

- Sexual urges, or behaviors involving touching and rubbing against a nonconsenting


person

Pedophilia

- Sexual urge or behaviors that involve sexual activity with a child or children
generally 13 years of age or younger
- The person is atleast 16 years of age and atleast 5 years older than the child or
children

Sexual masochism

- Sexual fantasies, urges, or behaviors involving the act of being humiliated, beaten,
restrained, or otherwise made to suffer

Sexual sadism

- Urges, or behaviors involving acts in which the psychological or physical suffering of


the victim is sexually exciting to the person.

Voyeurism

- Act of observing an unsuspecting person who is naked, in the process of disrobing, or


engaging in sexual activity
GENDER IDENTITY DISORDER
- A disorder which a person experience or pervasive feeling of discomfort with his/ her
own anatomic gender and has a constant desires to be a member of the opposite sex.
- Most common in men.

Diagnostic findings:

- Symptoms must be constant for not less than 2 years, not due to another mental
condition and not associated with genetic abnormality or physical intersexual disorder

Clinical manifestation:

- Fells uncomfortable in clothing designed for his/ her own sex


- Prefers clothes meant for opposite sex
- Daily activity are typical of the other sex
- Does not want to observe his/ her genitalia.

Treatment:

- Psychotherapy
- Behavior therapy
- Sex change operation
