10. Objective

The objective of the investigation is to find fact and the root cause as to why, where and how the
incident happened and also to advise the Diamond Kitchen Team to take corrective action in order to
prevent the same incident from occur again, also to develop recommendations for prevention of
injuries and illnesses to the company and worker.

11. Facts of the Case

On the above mention date, a huge fire had broken out around 1300 p.m. in Diamond Kitchen. It
spread so quickly that it consumed fairly a large number of kitchen tools and spare parts. The worst
part is, it caused damage to the entire restaurant. Above all, our chief chef, Mr Megat, and his co-
chef, Sofiyya was badly hurt and they were immediately sent to KPJ Damansara Specialist Hospital
for immediate medical treatment. Fire men were summoned and they extinguished the fire after
battling for about an hour.

Police was called in for their necessary action immediately after the incident. DOSH office was
informed via JKKP 107 form which was submitted in the morning by the immediate employer
(Diamond Kitchen).

12. Investigations

From interviews conducted on the witnesses and the inspection of the incident areas, the
investigation committee found that a container of canola oil being heated on the electric stovetop
boiled over and ignited. The chef doing the cooking turned off the burner and extinguished the fire
with a single portable ABC extinguisher but his efforts bring to no avail as the fire spread quickly.
Heat and heavy smoke activated a total of six detector heads in the kitchen, adjoining rooms, and the
hallway.
Due to the massive fire, the responsible parties strictly asked to close the restaurant for 60
days as all the apparatus need to be replaced. The estimated total lost of this incident is almost
Rm100,000.00

13. Recommendations

To prevent the similar incident from recurring, committee recommended to Diamond Restaurant

Management to take the following remedial and preventive actions:-

consider installation of an exhaust hood that discharges to the outside

consider installation of fixed extinguishing systems in full kitchens routinely used

for meal preparation

recommend mandatory fire safety training for employee

require additional fire safety training including hands on fire extinguisher training for

employee

15. Conclusion

Incident can be avoided if proper preventive measures are taken with full cooperation from everyone
to prevent the above incident. The middle management shall regularly promote Safety Awareness
Programme for workers by giving them regular safety talk.
By doing this workers will realize the importance of safety and in the end improving their
behavior, attitude and mentality towards safety.

We are of the opinion that this incident occurred due to Human Errors.
