Date: September 18th, 2017

On September 18th, 2017 Prestige Worldwide was performing out at the Atlantic Ocean on our
yacht when a guest fell off the side of the boat due to a gate not properly being closed. Prestige
Worldwide just recently received the sign on of Snoop Lion and received an anonymous
donation of $5,000,000,000.00. We hosted Snoop Lion and opened the yacht to friends and
business partners of Prestige Worldwide to celebrate our new partner.

Narrative of the Incident

The event launched from the port outside our recording studio in Los Angeles where 50+
attendees joined us in celebrating our partnership with Snoop Lion. To celebrate our new
partnership we decided to carter the event and provide an open bar. Before leaving the port, the
crews task was to secure the ship and close all gates and perform a safety check before the
vessel left the port. I spoke with Ship Captain, John Jones, and he was given a check list signed
off by his Crew Manager, Tom Pickles, that the safety check was performed before the ship left
the port.

The ship was authorized after the safety report to leave the port to kick off the celebration. While
out at sea, Prestige Worldwide, began with opening their hit song, Boats N Hoes. Snoop
Lion joined Prestige Worldwide, up on stage and had a speech prepared for his partnership.
During this time we had the ship crew, and guests gathered in the party hall of the yacht.

Immediately after the speech the ship captain was asking to speak to me in private. He informed
me that one of the guests had fallen off the side of the yacht. The ship was stopped and crew
members began searching for the guest. We were able to find, Ms. Ashley Dunn, on the right
side of the ship clinging onto a rope that was not properly stored away before departure.
Challenges of the Situation

After securing the guest and calling in the Coast Guard to diagnose the guest, we met with the
ship captain and crew. We took the safety check list and walked with the crew to review if the
check list was properly met. During the investigation it was clear that not every item was
checked properly. The ship captain pointed out that the rear gate was not properly secured and
was loose if leaned on. Also during the audit of the safety check list, we found that the ropes
were not properly stowed away after departure. Lastly, the manifest including the names of the
guests that were onboard was not signed off on which provided an inaccurate count of our
guests.

Possible Solutions

Going forward Prestige Worldwide will need to resolve these issues to prevent another incident.
To prevent further incidents, here are some solutions that should be enforced:

1. The captain must be present during the safety audit and while the check list is being
performed. To ensure that the check list and safety audit are being checked thoroughly.
2. The crew should be divided up and given specific duties with a supervisor to prevent
confusion on duties.
3. The vessel will perform a safety check-list and audit one hour before departure to ensure
that enough time is available to perform the checks.
4. Life boats attached to the main deck in case of an emergency, we suggest investing in at
least two of these boats to fit the guests and crew.
5. Paramedic on board during departures for medical and safety concerns.

Cost Analysis of Solutions

The proposed solutions will cost money, but will ensure the safety of our guests and crew for
further promotions and events. Prestige Worldwide must be able to provide strict safety
guidelines to prevent any further incidents. The cost of these solutions will cost, $450,000.00.
Here is a breakdown of the costs:

My Recommendations

I strongly suggest that we invest in life boats, and a more organized crew for ensuring the safety
of our guests and crew. If the yacht were to ever be in any danger of sinking or causing guests to
need to flee, we will have the proper equipment to provide their safety.

Our crew needs to be supervised and should be split into teams that will be given a specific task
according to the safety check list. That will help split the duties and if each team is supervised it
will enforce that the duty is done.

My last suggestion is that alcoholic beverages be limited to our guests, a two-drink policy will
stop our guests from any incident with intoxication. As the report on Ms. Ashley Dunn provided,
she was intoxicated and vomiting over the deck where the gate was NOT properly secured
causing her to fall. Enforcing this rule should prevent any incidents on intoxication.
