ASSESSMENT

In the morning in general hospital X, there is a patient who suffer stroke. In there
he have inpatient more than 3 days. He could not to act daily activity, because
right ecstremitas could not to move. Now, I will to do assessment about
immobilitation in connection with the stroke disease with the patient.

N : Good morning sir.

P : good morning sus.

N : how are you today mr.deris ?

Are you feel good ?

P : Im fine thank you sust. But, I dont feel so good, cause I could not to
move my right hand and my right leg.

N : ok, dont worry about it. I will to do assessment with you about it.

Are you agree ?to do it, I need more than ten minutes.

P : ok, I agree with you. When you do assessment, is it do you feel pain ?

N : I dont think so. So, you must relax. If you feel pain, please breath in

P :ow, ok sust thank you.

N : ok firstly, I want to explain this procedure. this assessment must to do
for to knew about your scalamusculo. Before, I start it, do you have any
questions ?

P : ok, thank you sust.

N : ok.

Now, can you lift your hand please now ?

P : I culdtsust.

N : umm, after I look it. youjust only lift your left hand, not right hand.

Then, can you shake your hands with me by your right hand ?

P : ough, I could not to move my hand.

N : please, keep calm mr.deris. Anything its ok.

N : can you to move your hand ?

P : Im sorry sust, I cant. I think, its very difficult to move it.

N : ok, now I will to check your leg.

Can you lift you leg please.

P : I can lift it, but not for my right leg. I think its very weight for to lift.

N : ok, I understand with your feel.

Then, if I touch your foot can you feel it ?

P : on my right foot I cant feel it.

N : then I will give you some questions.

What was this causes that you suffer stroke ?

P : umm, initially I have a hypertension, more than one year ago.

N : I dont think so about it, because I dont know complication of

hypertension.

Id like to suggest you that you must low salt food, cant depression, you
must bedrest for few days.

ok, I think its enough for this assessment. Then, maybe I will to practice
with buli-bulipanas so you feel comfortable.
