Pan EDitorial Thanks to the patronage of Oil & Gas Industry, the annual OISD conferenceonthe theme “Building Safety Culture through Inspiring Leadership” was a grand success. The conference attracted more than 275 participants; the quality of the papers and the presentations were such that even the last session - the valedictory session that concluded at around 1900 hours had a full house Earlier Minister of State for Petroleum & NG and leaders of the Oil industry organisations set the tone for the conference through their inspired and action oriented addresses. Our heartfelt thanks to all the participants, faculty members, session chairmen, senior officials from Ministry of PNG, Chairman - IOC, CMD - HPC, Board Level Directors of Oil & Gas companies, Hon'ble Minister of State for PNG for their unstinted support. Smt. Panabaka Lakshmi, Minister of State PNG, also released @ CD containing summary of all the papers presented during the conference. During the conference 19 papers were presented by the senior practising managers including one by Director-Marketing of OISD. The papers were very informative and were appreciated by the delegates. The presentation materials have been hosted at O'SD website www.0isd.nic.in The Hazira incident yet again points out that unless total respect is paid to the extant systems & procedures, accidents cannot be avoided. Each of our oll & gas companies have full-fledged comprehensive systems which if properly followed would ensure safe & trouble free operation. They are time and again violated. Let me once again relterate that marketing operation is not rocket science that would require extraordinary talents to manage safe & smooth operation but certainly deserves commitment and utmost vigil coupled with zero tolerance towards safety violations. The instructions that have been very clearly delineated in the operating manuals must be followed, One wonders why time and again we violate the SOPs. Is there any answer for the repetitive nature of accidents that are taking place due to disregard & disrespectto the systems? donot find any logical answer. Let us introspect OISD is now an ISO 9001:2008 organisation. It is obligatory on our part to streamline our own systems & procedures; we undertake audits of oil and gas installations in the country and therefore this endeavour for certification, OlSDians felt that the gaps which are there in the system must be liquidated for further strengthening our processes. The concerted effort of the team members enabled us to get ISO certification without any non- conformity. Thanks to the entire OISD team and in particular to Shri SK Bagchi, Additional Director and Management Representative for ISO. We have decided to become knowledge partners with Centre for Chemical Process Safety, USA with a view to providing a platform for sharing of knowledge & the latest developments in the field of process safety, exchanging experience, providing technical clarifications in safety matters, conducting process safety trainings, peer reviewing of each other's publications etc. MoU would be signed during mid-March, 2013. Hirak Dutta ee > Annual OISD Conference on Building Safety Culture in Organisation through Inspiring Leadership The Annual Oil Industry Safety Directorate (OISD) Conference on the theme “Building Safety Culture in Organization through Inspiring Leadership” was heldin Kolkata during 14-15 February, 2013. The conference was inaugurated by Hon'ble Minister of State for Petroleum & Natural Gas, Smt. Lakshmi Panabaka, in the gracious presence of Shri R.S. Butola, Chairman, IndianOil, Shri Subir Roychoudhury, C&MD, HPCL, Shri Hirak Dutta, Executive Director, OSD, Shri Raj Kumar Ghosh, Director (Refineries), Indian Oil and Ms, Nishi Vasudeva, Director (Marketing), HPCL Lighting ofthe lomp during the inaugural ceremony The conference was attended by more than 300 practicing managers in the oil & gas industry segment across India. Faculty members for the conference were drawn from leaders of oil & gas industry in India & abroad whoare thought leaders and policy makers. Shri Hirak Dutta, Executive Director, O1SD while welcoming the delegates, systemically analyzed the reasons for accidents that took place in the oil & gas industry. Shri Dutta mentioned that 75% of accidents happen due to disregard to the extant systems & procedures, poor supervisionand lack of operational knowledge. Therefore, he stated, it is important to develop a work culture that embraces employee engagement for achieving excellence in safety performance. Itisin this context the theme of this year’s conference has been selected 2s Building Safety culture through Transformational Leadership. Smt. Panabaka Lakshmi, Hon’ble Minister of State, MoP&.NG in her inaugural address dwelt upon the need to build a new work culture that fosters a climate of participation by employees at large. This would enhance safety performance, she added. Safe production must be our motto, mentioned Hon'ble Minister of State, She further added that safety performance of the company should be such that all stakeholders including the employees must take pride in being associated with the company having its only motto of: “Incident free production always”. MoS appreciated the good work carried out by OISD particularly in developing world-class safety standards and carrying Peirce ee Smt. Panabaka Lakshmi, MOS released the compendium of technical material out audits of various oil installations in the country. A compendium of conference technical material was released by Smt, Panabaka Lakshmi, MoSduring the occasion. Shri RS Butola, Chairman, !OC while delivering his address emphasized the need for change in attitude of people of the organization since the same machines and licensors are being used by the different oil companies but differing levels of excellence in safety management organization exists due to their work culture. He also emphasized the need to spend time with the shop-floor people to feel the difficulties and work environment so that corrective measures can be taken. Speaking candidly about Hazira Fire incident Shri Butola mentioned that complacency and overconfidence must be avoided to ensure safe operation Shri Subir Roychowdhury, CMD HPCL while addressing the audience emphasized the need for top driven approach in safety culture. He also advised the people that there isa need to look within ourselves to implement the approach towards safety by narrating a story from Mahatma Gandhi. While we have improved our standards, SOPs, systems & procedures, the human issue of safety culture is still an area of challenge lamented Shri Roychowdhury. Safety lies at the nucleus of the very existence of the organization and is an organizational necessity, he added. CMD, HPCL lauded OISD’s effort in propagating safety amongst the oil & gas industry. Shri RK Ghosh, Director (Refineries), IndianOil while delivering his address mentioned about the various incidents that occurred across the globe and their reasons. He reiterated that utmost vigil 24X7 is required to ensure accident free operation. Shri Ghosh spoke at length on strict management control that is required for enhancing safety in the oll & gas sector. We must also learn from national & international experiences, articulated Director-Refineries of 1OCL. The conference covered contemporary topics viz. Achieving process safety excellence thru benchmarking, Process safety instrumentation, Safety management & safety culture, Risk based inspection & fitness certification, Operational safety issues in offshore exploration & production, Concerns in marketing and its challenges and Issues and Challenges in CSR and sustainability Twenty one experts from India and abroad deliberated on the subjects and interacted with the delegates. The list of eminent speakers included from M/s Chevron, DuPont, MS Chola, Optra sensors, Essar, RIL, Cairn, BG, |OCL, HPCL, OISD, INCOIS, DNV, ABS etc. Ea C Meena anaes A view of delegates atterding the conference Shri LN Gupta, Joint Secretary, Ministry of Petroleum & NG in the valedictory session advocated incorporating safety as a performance parameter in the MoU of oil & gas companies. Shri Gupta felt that this would go a long way in furthering safety in vital hydrocarbon sector. Shri PK Singh, Dir. (SP&R),JS designate was alsopresent. Mr Hirak Dutta, ED, OISD made a brief presentation on the status of implementation. While reviewing the status Shri Bhargava advised all the members of Oil & Gas companies present to speed up the process of procurement and execution and cautioned them that any further delay in procurement and its execution will be dealt with firmly, While progress was made on procurement of items like DBBV, MFM, MEFG, HC detectors, oil marketing companies were found taking time to place purchase order for balance number of HVLR & Rim seal protection system. Oil companies were advised to quickly finalise and place PO for ROSOV and for those items for which the price part have been opened. ONGC & OIL was advised to pay focussed attention toprocurement items and submit the break-up of milestones twas observed that the refineries & pipelines achieved satisfactory progress in most of the areas. Thenext review meeting willbe heldin first week of January, 2013 Special Secretary, PNG reviewing the status on OISD-127 & MB Lal Committee recommendations With Senior Management executives of Oll& Gas Industry Sania Le SAFETY ALERT Electrocution due to violation of Work Permit System - RD. Danny & Parmod Kumar INTRODUCTION ; A fatal incident occurred at one of the refineries on 14th November 2012 at around 12:35 PM. During the incident a contractor employee got electrocuted at field while working on 6.6KV HT motor of Sulphur Recovery Unit (SRU). The deceased person received burn injuries from electrocution after coming in contact with the 6.6KV power source and was immediately shiftedto Refinery Hospital, where he wasdeciared brought dead ‘THE INCIDENT Main Combustion Chamber (MCC) air blowers-01/02/03 in SRU supplies airto the main burner to bum as much H25 as requiredto obtain 0.5 -0.7 vol. %H2S at the outlet of the 3rd reactor stage On the date of incident at around 01:30 AM, SRU shift engineer observed that MCC air blower-03 was not developing the required flow. Immediately, MCC blower-02 was started & taken in line to meet the process requirement. However, field operator couldn't stop MCC blower-03 from field. SRU shift engineer referred this problem to sub- station electrical operator, who in turn switched ‘OFF’ the MCC blower-03 breaker from6.6KV HT switchboard SRU shift engineer applied isolation for MCC blower-03 in electrical permit book kept in SRU control room. He then requested sub-station electrical operator to collect same from C/R& carry out necessary isolation from sub-station. Electrical operator didn't turn up to collect the isolation permit from SRU control room, and MCC blower-03 remained energized during night shift (6.6KV breaker 'OFF' butin 'Service' position). In morning shift, new SRU shift engineer misinterpreted the isolation status of MCC blower-03 & issued Clearance certificate (Work permit) to electrical maintenance for attending motor ‘field stop’ problem. Meantime, electrical testing group verbally requested electrical maintenance to disconnect power cables of MCC blower-03 for Diagnostic testing job through external agency. Electrical maintenance disconnected the field power cables of MCC blower-03, so that testing jobs can also be started simultaneously. After that electrical maintenance attended fieldstart/stop TNC switch, and it appears that while doing so they accidently ‘Closed’ the MCC blower-03 motor breaker from sub- station. With breaker closing, the power cable of MCC blower-03 got charged to6.6KV upto the motor terminal box. Deceased person approached the MCC blower-03 to note down the name plate details, and got electrocuted aftercomingin contact with charged power cables, +. Motor name plate located adjacent to cable terminal box. Motor cable terminal box - ‘open at the time of Incident. MCC air blower 03 HT motor & cable terminal box CRITICAL OBSERVATIONS ~ + Instructions regarding electrical isolation permit for MCC blower-03 & follow-up actions in morning shift were not clear in logbooks of SRU shift engineer & sub-station operators, on duty in Night shift. SRU shift engineer in Morning shift issued Work permit (clearance certificate) to electrical maintenance without actually confirming in writingthat electrical isolationhrasbeen carried out bysub-station operator. Itmay be noted that sub-station operator issue isolation certificate after ensuring breaker/switch is racked-out to ‘isolated! position, and thereby ruling outany possibilty of getting it ‘ON' from field push-button or localpanel Reason for not isolating MCC blower-03 in night shift itself couldn't be established, as there were contradictory statements from SRU production & Sub-station electrical operators. Asperthe extant procedure, after clearance certificate isissued by production department certifying equipment is electrically isolated, it is not mandatory that maintenance group shall also check & verify equipmentisolation status from sub-station. From the incident it was clear that when Electrical maintenance disconnected the power cables of MCC blower- 03, the 6.6KV breaker at sub-station end was still in ‘OFF! condition otherwise he would have been first to get electrical shock. Itis only while attendingthe field TNC (Trip-Neutral-Close) switch that the breaker switched 'ON’ from substation, and power cable got charged up to the motor terminal box. This possibly happened while E/M was tightening the control wirings & shorted the TNC ‘close’ contact, or while checking the freeness of TNC switch & moving it towards ‘close! position. Workpermit system was welldefinedin Refinery Safety manual Itwas found that safety audit of complete electrical system was carried out every year, but there was no random checks/review to validate compliance to work permit system by higher level officers. MCC blower-03 6.6KV feederfrom HT switchgear was tripped on earth fault protection The incident site was found little cramped. It appeared that while noting down the name plate details of MCC blower-03, contactor employee stood on blower discharge pipe & took support of motor terminal box. In the process, he got electrocuted after touching the exposed power cable accidently chargedto 6.6KV voltage. ROOT CAUSE ANALYSIS . + Production shift engineer while issuing the clearance certificate to electrical maintenance misinterpreted the breaker status (\solated/ OFF) of MCC blower-03 either due to unclear logbook instruction or miscommunication with sub-station operator. + Some of the maintenance activities related to incident were carried out without getting clearance certificate from production. Electrical maintenance disconnected the power cables of MCC blower-03, ‘though clearence certificate was issued to attend ‘field stop’ problem only. Electrical testing also requested power disconnection job directly to electrical maintenance instead of getting clearance from production first. + There was no back-up system against inadvertent operation due to wrong communication, poor job knowledge or human errors. Maintenance group should also exercise their control over hazardous power source by system like lock out and tag out (LOTO) wherein production & maintenance group provide individual lock out device (e.g. Pad Lock) at power source after verifying the isolation status, and ensuring no energisation can be done without their permission. Tag out device provides information about the nature of the lockout, and warns operating personnel not to energize electrical equipment. LOTO activity started after the incident MCC air blower-03 breaker in 6.6KV HT switchboard RECOMMENDATIONS + Strictadherence to standard operating procedures (SOPs) for work permit system to be ensured by working personnel of production & maintenance departments. + Before issuing clearance certificate (work permit), it must be ensured in writing that electrical isolation has been completed by isolating the breaker/ switches, and providing precautionary tags duly signed with date &time. + — Allshift activities & follow-up actions should be clearly recorded in Logbook. + — Electrical audits should include verification of electrical permit system & its compliance w.rt OISD-STD-105 by higher level officials more frequently, to ensure there is no deviation in the electrical isolation/ energisation activities. + — LOTO (lock out tag out) must be implemented wherein production & maintenance groups shall provide individual locking & tagging to ensure avoiding untoward incident & exercise control over the hazardous situations. 10 SAFETY ALERT E&P (Offshore) Title 2 Fire during Hot Bolting Location : Production platform Activity Type (Result/outcome] : Fire WHAT HAPPENED During hot bolting (the practice of removing and replacingor freeing and re-tightening bolts on live piping and equipment) of inlet valve flange on a glycol contact tower, an operator inadvertently struck and broke a grease fitting of the valve with a hammer. The fitting released high pressure gas intothe direction of the flame arrestor of line heater located approximately 10 feet from the contact tower's inlet valve. The gas ignited andwaseventually extinguished using dry chemical WHAT CAUSED IT Although a task-specific Job Safety Analysis (JSA) was conducted prior to initiating the hot bolting project, the following critical hazards and mitigating measures were not identified: + The fired component of the line heater was not shut-in prior to initiating the project. + The close proximity of the grease fitting was not identified as being a potential hazard, CORRECTIVE ACTIONS + Ina situation where there is a heat source in the immediate area with the possibility of flammable contents being released, hot bolting is not advised. A safer option would be to shut-in the effected equipment and bleed off all pressure before commencingthebolting operation + DuringJSA, specific consideration should be givento potential sources of release andignition points. + The JSA process should be used to review site-specific detailed job steps and uncover hazards associated with the specific job being undertaken + Operators should use onlynor-sparking tools when conducting hot bolting operations It doesn'tmatter how many resources you have Pata) OISD is now ISO 9001: 2008 certified organisation DNV auditors have recommended Oil Industry Safety Directorate, OISD, the technical wing under Ministry of Petroleum & NG, for ISO 9001; 2008 certification. Mr VK Khanna & Col. Anil the two Lead Auditors from M/s DNV undertook the audit on 21" December, 2012, The day long rigorous audit encompassed threadbare discussion with Top Management on quality systems of OISD followed by audit of all the functional areas viz, E&P, Refinery & Gas Processing, Marketing, Environment, HR, Materials and Finance. Opening meeting with DNV Auditors Earlier during the opening meeting with the lead auditors, a brief presentation on major OISD activities was made by Mr Sukhendu Majumdar, Director (MO). Shri Sk Bagchi, Additional Director and Management Representative for ISO mentioned that OISD is pursuing ISO certification for further strengthening its systems & practices. In the closing meeting with OlSDians, the lead auditors shared the findings of the audit and mentioned that M/s DNV is fully satisfied with the robust system & procedures and the customer centric approach of OSD. No major or minor non- conformities to the Quality System were observed bythe Lead Auditors ic (MO) sharing salient 0180 activities; aso seen on left MA, 150, CISD Hirak Dutta, Executive Director, OISD expressed deep satisfaction and mentioned that the purpose of the exercise was aimed at serving the stakeholders even better and therefore OISD pursued for ISO certification. Lauding the efforts of OlSDians in accomplishing this distinction, he urged that sustaining ISO Quality System is the responsibility of the entireteam and that OISD mustwork tirelessly to fulfil the mission of instillinga safety culture in theoil & gas industry to meet the ultimate goal of achieving highest safety standards, (OSD team with ENV Auditors Shri Sudhir Bhargava, Special Secretary and LN Gupta, Joint Secretary-Ref, Ministry of Petroleum & Natural Gas congratulated team OISD for being the first organisation under Ministry of PNG to get this distinction of ISO: 9001 certification. Dr. V Santosh, PS to Minister of State, PNG meets OISDians Dr. V Santosh was greeted by OlSDians on his arrival to Oil Industry Safety Directorate Office at Noida on February 3, 2013. This was the maiden visit of Or. Santosh to OISD after assuming the charge at Ministry of Petroleum & Natural Gas. Shri SkNandy, Adcitional Director, O1SD made brief presentation on various major activitiesof OISD. The presentation included the set-up of safety council and its advisory functions, organogram of OISD, aucits carried out, the numberof standards amended besides OISD's role in propagating knowledge on safety managementissues. Drawing from his experience on medical background, Dr. Santosh shared some of his views on measures to improve efficacy of Occupational Health services. Dr. Santosh appreciated besides others the services rendered by OlSDians in educating and imparting knowledge to member organisations on safety. Mr Hirak Dutta, Executive Director, OISD thanked Dr. Santosh for sharing his ideas for improving Occupational Health services and the interest he took in appreciating the major activities of CISD. Earlier Dr. Santosh was accorded with a warm floral welcome by ED, OISD upon hisarrival to OISD Conference Hall.
