Section 5.7
A) Positive Isolation is regarded as the most secure method and is to be considered in the development of all Isolation Plans.
Positive isolation shall be the isolation standard for:
1. Confined Space Entry into a vessel.
2. Hot work on piping and equipment excluding Hot work on steam, water or air systems below 2,000 kPag and below 200C (392F)
3. Boundary Isolations (eg. Brownfield ring fenced projects).
D) Other Isolation Devices such as mechanically expanded plugs, inflatable bags (stopple), frozen plugs (line freezing) or other non-
standard isolation devices, shall not be used as a primary form of isolation for hydrocarbon or hazardous service. OIDs shall be
treated as isolations methods with high potential risks. The level of isolation of OIDs is below that of single valve.
The purpose of the nonstandard OID isolation shall be clearly stated and shall have a designated design capability expressed in terms
of the pressure/temperature and leakage rate for the device or system provided by the designer/manufacturer of the device or
system.

2) All Permits used as part of the Isolation work shall be suspended by the Issuing Authority.
3) The Issuing Authority and Performing Authority shall sign the Sanction to Test section on the Isolation Plan and on the Work
Permit to authorize the activity.
4) The machine, equipment or process shall be cleared of the tools and materials. The work-site shall be cleared of personnel not
involved in the Sanction to Test.
5) The equipment shall be de-isolated according to the Sanction to Test procedure by IsA.
6) The Isolation Authority shall confirm that the correct isolation points have been de-isolated to allow the test to occur.
8) The equipment shall be de-energized and re-locked/tagged to continue the remaining work by IsA.
9) The Isolation Authority shall verify that the equipment has been isolated and de-energized
10) The Issuing Authority and Performing Authority shall revalidate the Permit to authorize the recommencement of the work.
