Task 1 :

My patient name is Mr Peter Catchart. His reason for admission in the hospital is Excessive
ketone production leading to suspected ketoacidosis. This is potentially life threatening
condition. With symptoms are abdominal pain, vomiting, rapid breathing, extreme tiredness,
adn drowsiness.

From that assessment, the intervention that I choose are monitoring the blood sugar levels, and
give an insulin injection to Mr Catchart. I also discuss diet that good for Mr catchart with a
dietician. I showed Mr Catchart how to Self-Monitoring Blood Glucose (SMBG). And of
course to demonstrating how to read SMBG stripe too. I Linking the SMBG to diet, and I
emphasized importance of monitoring blood sugar level every morning to Mr Carchart and
keep record of result to take to GP.

Task 2 :

Mr Herman Budiman, a patient 64 years old come to the hospital. His reason for admission is
an inflammatory process that occurs due to the deposition of uric acid crystals in the tissues
around the joints. With symptoms is pain in the right leg. Mr Budiman felt pain in his right leg
when he tried to walk as normal as he could do. Before this happen he had been diagnosed
Hypertension from doctor.

Nurse diagnosis is pain because of gout arthritis. The intervention that I take is to assess pain
(provoke, quality, radiation/relief, severity, and time). After the pain minimized , then observed
the reaction for comfort . collaborated with doctor about the Medicine Therapy. And at the end
checked the blood pressure again. After all the intervention is done we must make an
evaluation.

Task 3:

When I start my duty at the hospital, I must have checked everything that will be needed in a
good shape and ready to use. Prepare the room before the patient come and make the room's
comfort enough for patient . when the clinic is already open I called patient name's one by one
and then measure their vital signs like blood pressure and write it down on their document, and
give that paper to the doctor for the next pemeriksaan. I also give an information for a few
patient, what they must prepare before they insert their document to the interna clinic, like a
BPJS. Not only that, there was a patient that old enough that cant walk, so me and my friend
looking for a wheelchair for him sit and then accompany him to the laboratory. When there is
no remaining patient, we clean the clinic and cumulate all the patient document for today and
write it down on a journal book. When everything was finished I can go Home.
