(TPA) in the liberalized Health Insurance Sector. We are licensed as a TPA by the Insurance
Regulatory & Development Authority of India (License No: 005). As a TPA, we are an important
link between Insurance Companies, Policyholders & Healthcare Providers (Hospitals and Nursing
Homes).

We are offering the following services for Policyholders and Healthcare Providers:

Cashless Hospitalization: We enter into agreements with Healthcare Providers to offer

cashless and hassle free services to the Policyholders. Each Policyholder is provided with a list
of our empanelled Healthcare Providers wherein he/ she can avail this service.
ID card: We provide ID Cards to all our Policyholders in order to validate their identity at the
time of admission in any of our empanelled Healthcare Providers.
Claims Management: On behalf of the Insurance Companies, we administer and settle claims
for Healthcare Providers and Policyholders.
24 Hour Customer Support Services: MDIndia provides assistance through its 24 hours Call
Center and Help Desk. Information regarding Policyholders data, Provider Network.
AutoMail: Mail auto generated from the software gets sent to the Insurance Company /
Corporate, policyholder and Agent / Development Officer, Hospital.
AutoSMS: Hospital Generated through our software to acknowledge the receipt of all the
documents as well as for all the outward responses are sent to Insurance Company/Corporate,
Policyholder and Agent/Development Officer, Hospital.
Website: ID card and Claims status can be viewed by Policyholders, Providers and Corporates
from the website on online basis. www.mdindia.com
E Cards: For all enrolled members there is a provision of getting ID printed through the
website in case ID card is desired.
MDIndia Healthcare Services (TPA) Pvt. Limited would like to enter into an agreement with your
esteemed institution to offer Cashless Hospitalization to the Policyholders.

Please find attached a copy of our Provider Agreement, Provider Information Form and
Schedule of Charges. We request you to return the Provider Agreement signed & stamped
along with the Provider Information Form and Schedule of Charges duly filled.
