I, JAMIE C. BITUIN, of legal age, Filipino, single and

residing at 1561 A Kundiman St. Sampaloc, Manila, hereby
depose and say THAT:

1. I am currently a fourth year student of Perpetual

Help College of Manila, BS Business Administration.

2. I requested the school to allow me to wear casual

and maternity clothes for the entire second semester
of the School Year 2013-2014 due to my pregnancy.

3. I hereby undertake that I willingly enrolled to the

school despite my being 4 months pregnant and in
line with the same, I am removing all liabilities from
the school.

4. I am executing this Affidavit to attest to the

truthfulness of all the foregoing facts and for
whatever legal purposes this Affidavit may serve.

5. I am presently a professor at the University of Santo

Tomas for the course of Electronics Engineering.

6. I will be taking a study leave starting second

semester of the School Year 2012 -2013 or sometime
on November 2012.

7. I hereby undertake that I will not enter in any line of

work during the period of my study leave abroad.

8. I am executing this Affidavit to attest to the

truthfulness of all the foregoing facts and for
whatever legal purposes this Affidavit may serve.
