STUDENT INCIDENT REPORT

Incidents Date and Time: October 13, 2017 at around 1:30 P.M.
Students Name: Matthew Lewis B. Fajardo Grade & Section: 11-Justice (GAS)
Incidents Location: Principals Office
School Offense: Refusing to follow directions, using inappropriate language, employing
excessive and inappropriate attention-seeking behaviors and disrespect to authority

Incident's Description

It was around 1:30 PM the undersigned together with Mr. Mark Joseph Torres, the High
School Guidance Counselor, and Mrs. Teodocia L. Villanueva, the Principal, were talking to
John Michael Florentino of Grade 10-Faith regarding a confidential case when Matthew Lewis
B. Fajardo of Grade 11- Justice (GAS) entered the office of Mrs. Villanueva. He was asking for
his cellphone to be returned to him after it was collected by the Student Government officers
during the morning inspection. It has been a clear rule to the high school that cellphones which
will be confiscated (First Offense) has to be claimed by their parents or guardians. With these,
Mr. Fajardo burst to a very emotional state-shouting, telling everyone that he is not a criminal
and we are not supposed to get hold of his phone while uttering bad words. He was telling
everyone that he needs his phone since it was their anniversary of his girlfriend and he needs to
talk to him. He also needs to talk to a confidential man. Mr. Torres tried to pacify him, informing
him that he can borrow his phone if he really needs to use it for as long as he is only going to use
the phone inside the office and he will ask for it politely. And so he does.
The undersigned together with Mr. Torres and Matthew went to the High School Office
to get his phone. Matthew called someone using his phone. He asked Mr. Torres to accompany
him outside because he doesnt want the undersigned and Mr. Aldrin Flores, who was also in the
office, to hear what they will be talking about because its confidential. When they went out,
Matthew was shouting at Mr. Torres telling him that he was being treated like a criminal. Then
he run away from Mr. Torres uttering ipapatrabaho kita sir.
