To Whom It May Concern:

This is to certify that the following data was extracted from the Barangay Blotter of
Barangay, Dalimag, San Isidro, Abra.

Alleged Bee Sting Bite: On September 25,2017, Mrs. Catalina P. Malinnag and his
brother Ruben P. Pimentel came to request for Barangay Blotter, the incident that happened to
her husband last September 8, 2017, Miguel Josefino Malinnag, 60 years old, farmer, and
resident of Barangay Dalimag, San Isidro, Abra.

Miguel Josefino Malinnag was pasturing his Carabao at about 8:00 am in his farm called
Tucab, that all of a sudden hundreds of bees had bitten him. He shouted for help and his
brother in-law Ruben P. Pimentel who was cleaning also his rice field heard him shouting, so that
he went to help him, but the bees bite him, so he went home to Dalimag to ask for help. Upon
returning with his 5 companions, Miguel J. Malinnag, had moved already from the site for about
30 meters away and all the bees were gone.

That same day they brought the victim to Villaviciosa Medicare and community Hospital
for medical treatments, stayed for four days, then transferred to Abra Provincial Hospital for
further medical treatment where he again stayed for 6 days. They transferred again to Northside
Doctors Hospital in Guimod Bantay, Ilocos Sur, another further medical treatment.

The victim Miguel J. Malinnag was discharged from Northside Doctors Hospital on
September 23, 2017 at about 4:00 pm and brought home to Dalimag, San Isidro Abra. On the eve
of September 23, 2017 at about 10:15 pm his wife discovered he was dead.
