Re: DISCREPANCY IN NAME
We, ERMELINDA E. SEMETARA and JANET MARCOS, both of
legal age, Filipino and both with addresses at K74 Karangalan Drive,
Karangalan Village, Cainta, Rizal after having been duly sworn to in
accordance with law, depose and state that:

1. We personally know INNA CHRISTINE BUO, whose mother

PETULA BANAL BUO we have long been acquainted with, and the fact of
her birth is registered in the Office of the Civil Registrar in a Certificate of
Live Birth attesting thereto;

A copy of INNA CHRISTINE BUO Os Certificate of Live Birth

is hereto attached as Annex A.

2. We know for a fact that the person by the name of Inna Christina
Buno as appearing in the said Certificate of Live Birth and other documents,
and the name INNA CHRISTINE B. BUO as appearing in her school
records, refer to one and the same person, and that her true and correct
name is INNA CHRISTINE BUO.

Copies of the said school records bearing the name INNA

CHRISTINE B. BUO are hereto attached as Annex B-1 to B-4.

3. To avoid confusion as to the aforementioned names, we hereby

declare that INNA CHRISTINE BUO and INNA CHRISTINE B. BUO
refer to one and the same person, whose registered name is INNA
CHRISTINE BUNO.

4. We are not in any way related to the foregoing person within the
fourth civil degree either by consanguinity or affinity, nor do we have any
pecuniary interest in this matter whatsoever;

5, We are executing this affidavit to attest to the truth of the

foregoing facts and to establish the real identity of the aforementioned, for
all legal intents and purposes it may serve.
