• Attendance of Intern’s Reporting is a MUST
– EXCEPTION: Advised by From Duty resident to help in the OR
– Sundays and Holidays: 8AM for DUTY clerk
MAKE–UPS
o Please be on time
– Accumulated late >1 hour (whole rotation)
= number of hours late x 2
– Excused absence (with medical certificate)
= exact number of missed hours
– Unexcused absence = number of hours late x 2
o List of all those make–ups will be given at the
end of the year

o Pre-duty: 5PM (except for Saturdays,12NN)
o Post-duty: 5PM (except for Saturdays,12NN)

• Relay message of caller to your intern or resident first
before allowing conduction to Eye Center
• Do NOT conduct the following referrals
✓Trauma – laceration, fracture
✓Chemical injury – irrigation should be done at ER
• Ask for 1L PNSS, Kelly pad, MACROset
✓Unstable patients – high BP, non-ambulatory

– With Interns
• 2 COPIES of ER database:
✓ 1 for eye center and the other 1 for ER (use carbon paper)
• Complete all necessary data
• Indicate date, healthcard or if personal charge (whichever
is applicable) on upper right corner
• Check consultant for the day and write in the “Attending
Physician”
• Log ER patients in the logbook provided and on Excel file

- SOAP notes to be submitted at the end of rotation

– Eye Center technicians and staff will approach you if
there are any tasks needed

– Wear cap and mask inside (semi–sterile room)

each from the OPD (ideally new patients)
o DUTY clerks on MONDAY may also look for
cases (admitted/floor patients or ER patients)
o If no cases on Monday, TUESDAY pre-duty
and/or duty clerks will be tasked to look for
cases

o To be checked by Clerk’s Monitor or Resident
in charge by TUESDAY AFTERNOON
o Prepare powerpoint slide of all cases
