SIRE Inspection Preparation


It will greatly expedite the inspection if the following documents are available and ready for the Inspector’s
review on boarding:

1. Copies for the Inspector:


Prepared according to attached list on page 4.

2. The Inspector will wish to sight the following, where applicable:


The Certificate of Registry;
The vessel’s trading certificates, (the copies to be sighted if the originals are ashore): Safety Eqpmt,
Loadline, Safety Radio, IOPP, IAPP, Safcon, International Tonnage, ISM SMC & DoC, ISPS Cert., Certificate
of Fitness or Noxious Liquids Certificate; Sewage, Garbage & Antifouling Certificate
Oil Record Books Parts 1 and 2, Cargo Record Book; STS Manual, procedures & records.
Ballast Water & Garbage Mgmt. Plans and records. Bunker Operation procedures & records.
Certificates of Civil Liability for Oil Pollution, for Bunkers & for Wreck removal;
The USA Certificate of Financial Responsibility, and the last Tank Vessel Examination Letter;
Class Certificate, Enhanced Survey File w/Condition Assessment Report & quarterly class reports;
Approved manuals: Stability, Damage Stability, Loadicator, Inert Gas, COW, CBT, and ODME;
An approved SOPEP, VRP & VGP Manual; SOLAS Training/Maintenance Manual, VECS; VOC; SEEMP.
Lifesaving appliance and firefighting equipment maintenance records, drill plan & records;
The Procedures & Arrangements Manual, (for chemical and gas carriers); The Cargo Gear Register;
Officer & Crew national Competency Certificates, Continued Proficiency & Dangerous Cargo Endorsements;
Evidence of Administration acceptance of crew Certificates of Competency; ECDIS certification.
Near–Miss & Incident reporting, familiarization & training records.
The Manager’s Operating Instructions in general and internal inspection & audit records.
The Company Drug and Alcohol Policy, and records of unannounced testing;
Records of mooring winch brake tests, mooring rope/wire manufacturer’s & bow stopper certificate;
Mooring rope & wire certificates and condition reporting records. Hose certificates & test records.
The last 3 Port State Inspection certificates;
Hot work procedure, shoreside concurrence, permits as well as enclosed space entry permits;
The technical publications listed in the OCIMF Vessel Inspection Questionnaire (2000 Edition),
question 2.3, as applicable to the vessel.

It should be noted that the Emergency Fire Pump, Emergency Generator, Emergency Steering and the ODME
will be expected to be operated by the officer accompanying the inspector.

During the final meeting, a list of observations will be reviewed with the Master.
This review is to ensure that there are no observations that can be cleared because of a simple misunderstanding.
The Master is not expected to provide corrective actions or to indicate he agrees with the observations.
His signature is only an indication that he received the observations.

The Master can expect that the inspection will take about 6 to 8 hours and be performed in the following order,
unless the order of inspection needs be altered to avoid disruption of the ship’s normal operation & to fit the vessel’s schedule:

1. Review of above documentation (preparatory for Opening meeting)


2. Opening Meeting, with Master, Chief Engineer and Chief Mate (if available)
3. Complete review of documentation
4. Wheelhouse and navigation
5. Communications (radio room or GMDSS station)
6. The exterior of the Wheelhouse
7. The exterior of the Accommodations to the Poop
8. Up the Main Deck to the Forecastle and Forecastle spaces
9. Back down the Main Deck and checking a ballast tank (as agreed in Opening Meeting)
10. The Pump room
11. to the Cargo Control Room with Chief Officer
12. Discussion with Chief Engineer and into the Engine room including review of PMS.
13. With the Master back through the internal areas of the accommodations
14. Finally a period for the inspector to summarize his thoughts and a final meeting with the Master and Department heads.
